摘要
目的探讨分析早产儿凝血功能的改变与血清蛋白的相关性。方法方便选取该院新生儿科2017年1—12月收治的50例出生24 h内的早产儿,入院后给予防治早产儿并发症的一系列措施。在出生后24 h内以及治疗两周后分别测定前白蛋白及血清总蛋白的水平。结果治疗前早产儿的血清总蛋白(41.2±6.2)g/L及前白蛋白(50.7±10.4)mg/L的水平均低于治疗后,差异有统计学意义(t=19.731、74.166,P<0.05),胎龄较小的早产儿的血清总蛋白(40.3±3.3)g/L及前白蛋白水平(39.2±3.5)mg/L均低于胎龄较大的早产儿水平,差异有统计学意义(t=8.960、10.716,P<0.05),胎龄在32~37周的新生儿的凝血功能明显优于胎龄在28~32周的新生儿,差异有统计学意义(P<0.05)。血清白蛋白与PT、APTT、DDi、FDP负相关(r=-0.761、-0.782、-0.763、-0.745,P<0.05),与PLT正相关(r=0.731,P<0.05);前白蛋白与PT、APTT、DDi、FDP负相关(r=-0.713、-0.724、-0.735、-0.723,P<0.05),与PLT正相关(r=0.712,P<0.05)。结论早产儿的凝血功能与胎龄有关,而且还受到血清蛋白水平的影响。
Objective To explore the correlation between the changes of blood coagulation function and serum protein in premature infants.Methods A total of 50 premature infants who were admitted within 24 h after birth at the department of neonatology of the hospital from January to December 2017 were conveniently selected.After admission,a series of measures to prevent and prevent complications of premature infants were given.The levels of prealbumin and total serum protein were measured within 24 h after birth and two weeks after treatment.Results The levels of serum total protein(41.2±6.2)g/L and prealbumin(50.7±10.4)mg/L of premature infants before treatment were lower than after treatment,the difference was statistically significant(t=19.731,74.166,P<0.05),and the serum total protein of gestational age(40.3±3.3)g/L and prealbumin level(39.2±3.5)mg/L of younger preterm infants were lower than those of preterm infants with older gestational age,the difference was statistically significant(t=8.960,10.716,P<0.05),the coagulation function of newborns with gestational age of 32-37 weeks was significantly better than that of newborns with gestational age of 28-32 weeks,the difference was statistically significant(P<0.05).Serum albumin is negatively correlated with PT,APTT,DDi,FDP(r=-0.761,-0.782,-0.763,-0.745,P<0.05),and positively correlated with PLT(r=0.731,P<0.05);prealbumin was negatively correlated with PT,APTT,DDi,FDP(r=-0.713,-0.724,-0.735,-0.723,P<0.05),and positively correlated with PLT(r=0.712,P<0.05).Conclusion The coagulation function of premature infants is related to gestational age and is also affected by the level of serum protein.
