Taylor空间支架治疗胫腓骨中段及下段骨折的临床效果与安全性分析

摘要 目的:探讨Taylor空间支架(Taylor spatial frame,TSF)治疗胫腓骨中段及下段骨折的临床效果及安全性。方法:选取2015年7月-2019年1月笔者所在医院收治的90例胫腓骨中段及下段骨折患者,采用回顾分析法,根据不同骨折固定治疗将其分为内固定组40例及外固定组50例。两组患者均由同一组医师完成手术,内固定组患者进行手法复位及应用点状复位钳间接复位治疗,外固定组患者进行Taylor空间支架治疗,并比较两组患者手术情况、固定物去除时间及骨折愈合时间。观察并比较两组患者术后并发症、患肢功能情况。结果:外固定组患者手术时间、固定物去除时间及骨折愈合时间均短于内固定组,术中出血量少于内固定组(P<0.05);外固定组患者术后出现骨折延迟愈合1例,软组织缺损2例,内固定组患者出现钢板外露1例,钢板断裂1例,软组织缺损1例,两组术后并发症发生率比较差异无统计学意义(P>0.05);外固定组患者患肢功能优良率96.00%,内固定组患者患肢功能优良率87.50%,两组患肢功能优良率比较差异无统计学意义(P>0.05)。结论:Taylor空间支架治疗胫腓骨中段及下段骨折临床效果显著,并发症少,相较于内固定钢板治疗手术时间、固定物去除时间及骨折愈合时间更短,缩短骨折治疗康复周期,有利于促进患者患肢功能恢复,安全性好,较为可靠。 Objective:To observe the clinical efficacy of Taylor spatial frame in the treatment of middle and lower tibiofibular fractures.Method:From July 2015 to January 2019,90 patients with middle and lower tibia and fibula fractures were selected.The patients were divided into 40 cases of the internal fixation group and 50 cases of the external fixation group according to different fracture fixation.Both groups were operated by the same group of physicians.The internal fixation group was treated with manual reduction and indirect reduction with punctate reduction forceps,while the external fixation group was treated with Taylor spatial frame.The operative situation,fixation removal time and fracture healing time were observed and compared between the two groups.The postoperative complications,limb function were observed and compared between the two groups.Result:The time of operation,the time of removal of fixations and the time of fracture healing shorter than those of internal fixation group (P<0.05).There were 1 cases of delayed fracture healing,2 cases of soft tissue defect in the external fixation group;1 case of exposed plate,1 case of broken plate and 1 case of soft tissue defect in the internal fixation group,the difference of complication rate between the two groups was no statistically significant (P>0.05).The excellent and good rate of limb function in the external fixation group was 96.00%,the excellent and good rate of limb function in the internal fixation group was 87.50%,the difference was no statistically significant (P>0.05).Conclusion:Compared with the internal fixation plate,the operation time,the removal time and fracture healing time of spatial frame treatment are shorter and the complications are less,and the recovery period of fracture treatment is shortened.It is beneficial to promote the recovery of limb function and lower limb force line.
