摘要
目的:探讨MRI对卵巢上皮性肿瘤的诊断价值。方法:选取2019年2月-2021年2月笔者所在医院收治20例卵巢上皮性恶性肿瘤患者(恶性组)和42例良性肿瘤患者(良性组)进行MRI检查。观察两组MRI诊断准确率、扫描表现、ADC值及信号表现。结果:MRI诊断良性肿瘤40例,准确率为95.2%(40/42),恶性肿瘤19例,准确率为95.0%(19/20);恶性组肿块直径>4 cm、肿块囊实性、肿块囊内分隔厚度>3 mm、肿块见结节状或菜花状突起及肿块坏死率高于良性组(P<0.05);恶性组囊性部分ADC值与良性组比较差异无统计学意义(P>0.05),恶性组实性部分ADC值低于良性组(P<0.05);恶性卵巢上皮性肿瘤囊壁多呈厚而不均匀,边界模糊,T1WI低信号,T2WI高信号,良性则囊壁薄而均匀,边界清晰,T1WI低信号,T2WI低信号,恶性组与良性组的囊壁、边界及T2WI比较差异有统计学意义(P<0.05)。结论:依据MRI的扫描特点及信号表现,在鉴别卵巢上皮性肿瘤中具有较高的诊断准确率,为后期诊疗提供一定的临床依据,值得推广。
Objective:To evaluate the diagnostic value of MRI in epithelial ovarian tumors.Method:Twenty patients with epithelial ovarian malignant tumors and forty-two patients with benign tumors admitted to our hospital from February 2019 to February 2021 were selected for MRI examination and compared with pathology.MRI diagnostic accuracy,scanning performance,ADC value and signal performance were observed and analyzed.Result:The diagnostic accuracy of MRI was 95.2% (40/42) in 40 cases of benign tumors and 95.0% (19/20) in 19 cases of malignant tumors.Compared with the benign group,the tumor diameter >4 cm,the cystic solid mass,the intracapsular partition thickness >3 mm,the nodular or cauliflower protrusion and the necrosis rate of tumor in the malignant group were higher than those in the benign group (P<0.05).There was no difference in the cystic part of ADC between the malignant group and the benign group (P>0.05),and the solid part of ADC in the malignant group was lower than that in the benign group (P<0.05).The cyst wall of malignant epithelial ovarian tumors was thick and uneven,with fuzzy boundary,low signal on T1WI and high signal on T2WI,while the cyst wall of benign tumors was thin and uniform,with clear boundary,low signal on T1WI and low signal on T2WI.There were statistically significant differences in cyst wall,boundary and T2WI between malignant group and benign group (P<0.05).Conclusion:Compared with pathological control,MRI has higher diagnostic accuracy in differentiating epithelial ovarian tumors based on its scanning characteristics and signal performance,which provides certain clinical basis for later diagnosis and treatment,and is worthy of promotion.
