鼻骨骨折122例临床分析

Clinical Analysis of 122 Cases of Nasal Bone Fracture

目的:探讨鼻骨骨折的治疗情况。方法:回顾性分析北京市石景山医院2018年10月-2020年10月122例鼻骨骨折患者治疗情况。根据患者骨折类型、病情需要和个人意愿,在鼻部伤后7~10 d,待鼻部消肿后再进行处理骨折,最晚不超过14 d。观察各临床分型鼻骨骨折的治疗情况。结果:Ⅰ型24例,Ⅱ型60例,Ⅲ型20例,Ⅳ型18例。84例患者全部一次性临床治愈,其中60例行闭合性鼻骨骨折复位术,24例行鼻内镜下鼻骨骨折复位;24例中11例同期在鼻内镜下行鼻中隔成形术。结论:近一半的鼻骨骨折为Ⅱ型。闭合性鼻骨骨折复位术可解决约50%鼻骨骨折的外鼻畸形。鼻内镜下鼻骨骨折复位同时行鼻中隔成形术既可以矫正鼻腔畸形又可以改善鼻腔通气引流。

Objective:To discuss the treatment of nasal bone fractures.Method:A retrospective analysis of the diagnosis and treatment of 122 patients with nasal bone fractures in Beijing Shijingshan Hospital from October 2018 to October 2020.According to the type of fracture,the needs of the patients’ condition and personal wishes,the fracture should be treated 7-10 days after the nose injury and after the swelling of the nose,no more than 14 days at the latest.Different clinical types of nasal bone fracture treatment were observed.Result:There were 24 cases of type Ⅰ,60 cases of type Ⅱ,20 cases of type Ⅲ,and 18 cases of type Ⅳ.All 84 patients were clinically cured at one time,60 cases underwent closed nasal bone fracture reduction,and 24 cases underwent endoscopic nasal bone fracture reduction;11 of 24 cases underwent nasal septum plasty under nasal endoscopy at the same time.Conclusion:Nearly half of nasal bone fractures are type Ⅱ.Closed nasal bone fracture reduction can solve about 50% of the external nasal deformities of nasal bone fractures.Endoscopic nasal bone fracture reduction and nasal septum plasty can not only correct nasal cavity deformities but also improve nasal ventilation and drainage.
