神经肌肉电刺激联合血流限制训练在前交叉韧带重建术后早期康复中的应用疗效 被引量:7

Effect of Neuromuscular Electrical Stimulation Combined with Blood Flow Restriction Training in Early Rehabilitation after Anterior Cruciate Ligament Reconstruction

摘要 目的:探讨神经肌肉电刺激联合血流限制训练在前交叉韧带重建术后早期康复中的应用疗效。方法:选择2017年6月-2020年6月在笔者所在医院行前交叉韧带重建术治疗的患者82例,按照随机数字表法分为对照组和观察组,各41例。对照组患者在术后接受常规康复治疗联合神经肌肉电刺激治疗,观察组患者在对照组基础上加以采用血流限制训练。比较两组患者康复效果、关节活动度、关节活动舒适度及功能、股四头肌等速肌力、大腿周径、并发症发生情况。结果:观察组患者康复优良率明显高于对照组(P<0.05)。治疗后2、4、6周,两组关节活动度均明显高于治疗前,且观察组均明显高于同期对照组(P<0.05)。治疗后,两组Lysholm评分明显高于治疗前,且观察组明显高于对照组,两组VAS评分明显低于治疗前,且观察组明显低于对照组(P<0.05)。治疗后,两组峰力矩、总功值、平均功率水平均明显高于治疗前,且观察组明显高于对照组(P<0.05)。两组并发症发生率比较差异无统计学意义(P>0.05)。结论:采用神经肌肉电刺激联合血流限制训练康复治疗,有利于促进行前交叉韧带重建术治疗患者术后后早期康复,减轻膝关节疼痛及肿胀,改善膝关节日常活动以及运动功能,效果安全可靠。 Objective:To explore the effect of neuromuscular electrical stimulation combined with blood flow restriction training in the early rehabilitation after anterior cruciate ligament reconstruction.Method:A total of 82 patients who underwent anterior cruciate ligament reconstruction in our hospital from June 2017 to June 2020 were selected and divided into the control group and the observation group according to a random number table,with 41 cases in each group.Patients in the control group received conventional rehabilitation therapy combined with neuromuscular electrical stimulation treatment after the operation,and patients in the observation group received blood flow restriction training on the basis of the control group.The rehabilitation effect,joint range of motion,comfort and function of joint motion,isokinetic strength of quadriceps,thigh circumference,occurrence of complications were compared between the two groups of patients.Result:The recovery rate of patients in the observation group was significantly higher than that in the control group (P<0.05).At 2,4,and 6 weeks after treatment,the joint range of motion in the two groups was significantly higher than before treatment,and the observation group was significantly higher than the control group in the same period (P<0.05).After treatment,the Lysholm scores of the two groups were significantly higher than those before the operation,and the observation group was significantly higher than the control group,the VAS scores of the two groups were significantly lower than that before the operation,and the observation group was significantly lower than the control group (P<0.05).After treatment,the peak torque,total work value,and average power levels of the two groups were significantly higher than before treatment,and the observation group was significantly higher than the control group (P<0.05).There was no significant difference in the incidence of complications between the two groups (P>0.05).Conclusion:The use of neuromuscular electrical stimulation combined with blood flow restriction training and rehabilitation therapy is conducive to promoting the early postoperative rehabilitation of patients undergoing anterior cruciate ligament reconstruction,reducing the pain and swelling of the knee joint,improving the daily activities and motor function of the knee joint,and the effect is safe and reliable.
