玉屏风散合针灸治疗变应性鼻炎的效果及对鼻腔灌洗液黏附分子、TXB2和白三烯水平的影响

Effect of Yupingfeng powder and acupuncture in treatment of allergic rhinitis and its influence on levels of nasal lavage fluid adhesion molecules,thromboxane B2 and leukotrienes

目的探讨玉屏风散合针灸治疗变应性鼻炎的效果及对鼻腔灌洗液黏附分子、血栓素B2(TXB2)和白三烯水平的影响。方法选取2016年10月⁃2018年1月河南省中医院耳鼻喉科收治的98例变应性鼻炎患者为研究对象,采用随机数字表法分为对照组和观察组各49例,对照组实施针灸疗法治疗,观察组实施玉屏风散合针灸治疗,比较两组临床疗效,观察治疗前后两组鼻腔灌洗液黏附分子ICAM⁃1、IL⁃6、TXB2、白三烯水平变化及临床症状改善情况。结果治疗后观察组临床总有效率较对照组明显高,差异有统计学意义(P<0.05);治疗前两组ICAM⁃1、IL⁃6、TXB2、白三烯、鼻塞、流涕、鼻痒、喷嚏评分相较无明显差异(P>0.05);与治疗前相较,治疗后两组ICAM⁃1、IL⁃6、TXB2、白三烯、鼻塞、流涕、鼻痒、喷嚏评分明显降低(P<0.05),且治疗后观察组ICAM⁃1、IL⁃6、TXB2、白三烯、鼻塞、流涕、鼻痒、喷嚏评分较对照组明显低,差异有统计学意义(P<0.05)。结论玉屏风散合针灸治疗变应性鼻炎的效果明确,可有效缓解鼻腔灌洗液黏附分子、TXB2和白三烯水平,同时明显改善患者临床症状,有一定临床推广应用价值。

Objective To investigate the effect of Yupingfeng powder and acupuncture in the treatment of allergic rhinitis and its influence on the levels of nasal lavage fluid adhesion molecules,thromboxane B2(TXB2)and leukotrienes.Methods TNinety⁃eight patients with allergic rhinitis admitted to the Department of Otolaryngology in Henan Provincial Hospital of Traditional Chinese Medicine between October 2016 and January 2018 were included as the subjects in the study.All patients were randomly divided into the control group and the study group(n=49 each).The control group was treated with acupuncture,and the study group was treated with Yupingfeng powder and acupuncture.The clinical effect was compared between the two groups.The changes in the levels of nasal lavage fluid adhesion molecules[intercellular adhesion molecule⁃1(ICAM⁃1),interleukin⁃6(IL⁃6)],TXB2 and leukotrienes and the improvement of clinical symptoms in both groups at the baseline and after the treatment were determined.Results After the treatment,the clinically total effective rate in the study group was significantly higher than that in the control group,with statistically significant difference(P<0.05).There were no significant differences in the ICAM⁃1,IL⁃6,TXB2,leukotrienes,nasal obstruction,running nose,rhinocnesmus and sneezing scores between the two groups at the baseline(P>0.05).The ICAM⁃1,IL⁃6,TXB2,leukotrienes,nasal obstruction,running nose,rhinocnesmus and sneezing scores significantly decreased after the treatment compared with those at the baseline(P<0.05).The ICAM⁃1,IL⁃6,TXB2,leukotrienes,nasal obstruction,running nose,rhinocnesmus and sneezing scores in the study group were significantly lower than those in the control group after the treatment,with statistically significant differences(P<0.05).Conclusion Yupingfeng powder and acupuncture offer definite therapeutic efficacy in allergic rhinitis by effectively improving the levels of nasal lavage fluid adhesion molecules,TXB2 and leukotrienes,and significantly alleviating the clinical symptoms,which justifies its widespread use in the clinical settings.
