社区督导员督导管理、访视在非住院肺结核患者中的应用效果

Application effect of community supervisor's supervision management and visit in non hospitalized patients with pulmonary tuberculosis

目的探讨社区督导员督导管理、访视在非住院肺结核患者中的应用效果。方法选取2018年7月至2019年3月100例非住院肺结核患者,按照随机数字表法将其分为对照组和观察组,各50例。对照组实施常规督导,观察组实施社区督导员督导管理、访视。比较两组的健康素养评分、心理状态评分、服药依从性、痰菌转阴率、病灶吸收率、空洞闭合率、生活质量评分、满意度。结果干预后,观察组的健康知识、健康信念、健康行为、健康技能评分高于对照组(P<0.05)。干预后,观察组的SAS、SDS评分低于对照组(P<0.05)。观察组的服药依从率、痰菌转阴率、病灶吸收率、空洞闭合率高于对照组(P<0.05)。干预后,观察组的生理、心理、环境、社会关系评分高于对照组(P<0.05)。观察组的满意度高于对照组(P<0.05)。结论社区督导员督导管理、访视可提高非住院肺结核患者的健康素养,改善心理状态,提高服药依从性,促进痰菌转阴、病灶吸收、空洞闭合,提高生活质量及满意度。

Objective To explore the application effect of community supervisor's supervision management and visit in non hospitalized patients with pulmonary tuberculosis.Methods One hundred non hospitalized patients with pulmonary tuberculosis from July 2018 to March 2019 were selected and divided into control group and observation group according to random number table method,with 50 cases in each group.The control group implemented routine supervision,and the observation group implemented community supervisor's supervision management and visit.The health literacy score,mental state score,medication compliance,sputum negative conversion rate,lesion absorption rate,cavity closure rate,quality of life score and satisfaction were compared between the two groups.Results After intervention,the scores of health knowledge,health belief,health behavior and health skills in the observation group were higher than those in the control group(P<0.05).After intervention,the scores of SAS and SDS in the observation group were lower than those in the control group(P<0.05).The medication compliance rate,sputum negative conversion rate,lesion absorption rate and cavity closure rate in the observation group were higher than those in the control group(P<0.05).After intervention,the scores of physiology,psychology,environment and social relations in the observation group were higher than those in the control group(P<0.05).The satisfaction of the observation group was higher than that of the control group(P<0.05).Conclusion Community supervisor's supervision management and visit can improve the health literacy of non hospitalized patients with pulmonary tuberculosis,ameliorate psychological state,improve medication compliance,promote the sputum negative conversion,lesion absorption and cavity closure,and improve the quality of life and satisfaction.
