1470 nm激光经尿道膀胱肿瘤剜除术和经尿道膀胱肿瘤电切术治疗非肌层浸润性膀胱肿瘤的疗效比较

Comparison on the efficacy of 1470 nm laser transurethral bladder tumor enucleation and transurethral resection of bladder tumor in the treatment of non-muscular invasive bladder tumors

目的比较1470 nm激光经尿道膀胱肿瘤剜除术和经尿道膀胱肿瘤电切术治疗非肌层浸润性膀胱肿瘤的疗效。方法90例非肌层浸润性膀胱肿瘤患者,采用随机数字表法分为观察组和对照组,每组45例。观察组采用1470 nm激光经尿道膀胱肿瘤剜除术治疗,对照组采用经尿道膀胱肿瘤电切术治疗。比较两组围手术期指标、术后相关并发症发生情况。结果观察组手术时间(23.9±5.4)min、留置导尿管时间(1.6±0.5)d短于对照组的(33.8±8.6)min、(2.4±0.7)d,术中出血量(16.8±3.2)ml少于对照组的(23.4±4.8)ml,差异具有统计学意义(P<0.05);两组住院时间比较差异无统计学意义(P>0.05)。观察组闭孔神经反射发生率2.22%低于对照组的13.33%,差异具有统计学意义(P<0.05);两组膀胱穿孔、尿道狭窄发生率比较差异无统计学意义(P>0.05)。结论1470 nm激光经尿道膀胱肿瘤剜除术治疗非肌层浸润性膀胱肿瘤,相对于经尿道膀胱肿瘤电切术治疗,可缩短手术时间,减少手术创伤,降低术后相关并发症发生率。

Objective To compare the efficacy of 1470 nm laser transurethral bladder tumor enucleation and transurethral resection of bladder tumor in the treatment of non-muscular invasive bladder tumors.Methods A total of 90 patients with non-muscular invasive bladder tumors were divided into observation group and control group according to random numerical table,with 45 cases in each group.The observation group was treated with 1470 nm laser transurethral bladder tumor enucleation,and the control group was treated with transurethral resection of bladder tumor.The perioperative indicators and the incidence of postoperative complications were compared between the two groups.Results The operation time(23.9±5.4)min and catheter indwelling time(1.6±0.5)d of the observation group were shorter than(33.8±8.6)min and(2.4±0.7)d of the control group,and the intraoperative blood loss(16.8±3.2)ml was less than(23.4±4.8)ml of the control group.All the difference was statistically significant(P<0.05).There was no statistically significant difference in hospitalization time between the two groups(P>0.05).The incidence of obturator nerve reflex of the observation group was 2.22%,which was lower than 13.33%of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in incidences of bladder perforation and urethral stricture between the two groups(P>0.05).Conclusion Compared with transurethral resection of bladder tumors,1470 nm laser transurethral bladder tumor enucleation for non-muscular invasive bladder tumors can shorten the operation time,reduce surgical trauma,and reduce the incidence of postoperative complications in patients.
