探讨月经紊乱患者血清性激素与甲状腺功能的相关性分析 被引量:3

摘要 目的分析月经紊乱患者血清性激素与甲状腺功能的相关性。方法选取80例月经紊乱患者为观察组,另选取80例接受健康检查的受检者为对照组。两组均进行血清性激素与甲状腺功能检测,分析检测结果。结果观察组酮(T)、孕酮(P)、雌二醇(E2)、催乳素(PRL)、促卵泡成熟激素(hFSH)、促黄体生成素(hLH)分别为(0.62±0.31)pg/ml、(1.78±0.99)μg/ml、(69.52±3.13)pg/ml、(18.92±7.48)ng/ml、(12.02±1.48)mIU/ml、(14.59±2.42)mIU/ml,对照组分别为(0.66±0.32)pg/ml、(1.82±0.13)μg/ml、(88.49±5.19)pg/ml、(14.12±6.39)ng/ml、(10.76±1.39)mIU/ml、(11.26±2.18)mIU/ml;两组T、P水平比较差异无统计学意义(P>0.05),观察组E2水平低于对照组,PRL、hFSH、hLH水平均高于对照组,差异有统计学意义(P<0.05)。观察组三碘甲状腺原胺酸(T3)、甲状腺素(T4)、游离T3(FT3)、游离T4(FT4)、促甲状腺素(TSH)水平分别为(2.19±0.68)nmol/L、(121.43±16.23)nmol/L、(6.28±2.11)pmol/L、(16.73±2.98)pmol/L、(2.81±1.05)IU/ml,对照组分别为(2.12±0.63)nmol/L、(118.76±15.29)nmol/L、(6.35±2.08)pmol/L、(16.53±2.79)pmol/L、(2.76±1.02)IU/ml;两组T_(3)、T_(4)、FT_(3)、FT_(4)、TSH水平比较差异无统计学意义(P>0.05)。观察组中亚临床甲状腺功能减退症(亚甲减)与甲状腺功能减退症(甲减)T、P、E2、PRL、hFSH、hLH均低于亚临床甲状腺功能亢进症(亚甲亢)与甲状腺功能亢进症(甲亢),且亚甲减高于甲减、亚甲亢低于甲亢,差异有统计学意义(P<0.05)。结论月经紊乱患者性激素水平会出现异常改变,且甲状腺功能异常发生率较高,甲状腺功能不同异常类型患者性激素水平差异明显,测定甲状腺功能可辅助诊断月经紊乱。 Objective To analyze the correlation between serum sex hormone and thyroid function in patients with menstrual disorder.Methods There were 80 patients with menstrual disorder as the observation group,and 80 healthy subjects as the control group.Both groups were tested for serum sex hormones and thyroid function,and the test results were analyzed.Results The testosterone(T),progesterone(P),estradiol(E2),prolactin(PRL),human follicle stimulating hormone(hFSH),human luteinizing hormone(hLH)of the observation group were(0.62±0.31)pg/ml,(1.78±0.99)μg/ml,(69.52±3.13)pg/ml,(18.92±7.48)ng/ml,(12.02±1.48)mIU/ml and(14.59±2.42)mIU/ml,which were(0.66±0.32)pg/ml,(1.82±0.13)μg/ml,(88.49±5.19)pg/ml,(14.12±6.39)ng/ml,(10.76±1.39)mIU/ml and(11.26±2.18)mIU/ml of the control group.There was no statistically significant difference in T and P levels between the two groups(P>0.05).The E2 level of the observation group was lower than that of the control group,and the levels of PRL,hFSH and hLH were higher than those of the control group.All the difference was statistically significant(P<0.05).The levels of triiodothyronine(T3),thyroxine(T4),free T3(FT3),free T4(FT4),thyroid stimulating hormone(TSH)of the observation group were(2.19±0.68)nmol/L,(121.43±16.23)nmol/L,(6.28±2.11)pmol/L,(16.73±2.98)pmol/L and(2.81±1.05)IU/ml,which were(2.12±0.63)nmol/L,(118.76±15.29)nmol/L,(6.35±2.08)pmol/L,(16.53±2.79)pmol/L and(2.76±1.02)IU/ml of the control group.There was no statistically significant difference in levels of T_(3),T_(4),FT_(3),FT_(4) and TSH between the two groups(P>0.05).In the observation group,the T,P,E2,PRL,hFSH,and hLH of subclinical hypothyroidism and hypothyroidism patients were lower than those of sub-clinical hyperthyroidism and hyperthyroidism patients,and sub-clinical hyperthyroidism patients were higher than sub-clinical hypothyroidism patients,and sub-clinical hyperthyroidism patients were lower than hyperthyroidism patients.The difference was statistically significant(P<0.05).Conclusion The level of sex hormones in patients with menstrual disorders will change abnormally,and the incidence of abnormal thyroid function is relatively high.There are obvious differences in the levels of sex hormones in patients with different types of abnormal thyroid function,and the measurement of thyroid function can assist in the diagnosis of menstrual disorders.
