阴道产钳助产对产后盆底功能的影响及康复治疗的效果评价

Effect of forceps midwifery on postpartum pelvic floor function and evaluation of the effect of rehabilitation therapy

目的研究阴道产钳助产对产妇盆底功能的影响,并分析产后康复治疗在产钳助产在产妇盆底功能、盆底肌力恢复中的应用价值与治疗效果。方法100例分娩产妇,抽签将其分为对照组和实验组,每组50例。对照组进行自然分娩,实验组进行产钳助产,产后8周均进行康复治疗。比较两组产妇康复治疗前和康复治疗后2个月的盆底功能、治疗效果以及不良反应发生情况。结果实验组产妇分娩后康复治疗前前静息平均肌电值、快肌最大肌电值、混合肌平均肌电值、慢肌平均肌电值、后静息平均肌电值均低于对照组,差异均具有统计学意义(P<0.05)。康复治疗后,两组前静息平均肌电值、快肌最大肌电值、混合肌平均肌电值、慢肌平均肌电值、后静息平均肌电值比较,差异均无统计学意义(P>0.05)。两组康复治疗总有效率比较,差异无统计学意义(P>0.05)。实验组不良反应发生率24%略高于对照组的18%,但差异无统计学意义(P>0.05)。结论阴道产钳助产对产妇盆底功能影响较大,进行产后康复治疗后盆底功能有明显改善,因此,康复治疗在产后盆底功能恢复中具有较高的应用价值。

Objective To study the effect of forceps midwifery on pelvic floor function of puerpera,and analyze the practical value and therapeutic effect of postpartum rehabilitation therapy in pelvic floor function and pelvic floor muscle strength recovery of puerpera during forceps-assisted vaginal delivery.Methods A total of 100 puerpera were divided into control group and experimental group according to lottery method,with 50 cases in each group.The control group underwent vaginal delivery,and the experimental group underwent forceps midwifery.Both groups received rehabilitation treatment 8 weeks after delivery.The pelvic floor function before rehabilitation treatment and 8 weeks after rehabilitation treatment,therapeutic effect and occurrence of adverse reactions were compared between the two groups.Results Before rehabilitation treatment,the average electromyography value of pre-resting,the maximum electromyography value of fast muscle,the average electromyography value of mixed muscle,the average electromyography value of slow muscle and the average electromyography value of post-resting in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).After rehabilitation treatment,there was no statistically significant difference in the average electromyography value of pre-resting,the maximum electromyography value of fast muscle,the average electromyography value of mixed muscle,the average electromyography value of slow muscle and the average electromyography value of post-resting between the two groups(P>0.05).There was no significant difference in the total effective rate of rehabilitation treatment between the two groups(P>0.05).The incidence of adverse reactions 24%in the experimental group was a little higher than 18%in the control group,and the difference was statistically significant(P<0.05).Conclusion Forceps midwifery has a great influence on the pelvic floor function of puerpera,and the pevic floor function can be significantly improved after postpartum rehabilitation.Therefore,rehabilitation therapy has a high application value in postpartum basin function recovery.
