酒精依赖患者的脑白质病变分级及其与血清神经丝轻链蛋白及患者认知功能的相关性研究 被引量:4

A Preliminary Study on White Matter Lesion Severity of Alcohol Dependence Patients and Their Correlation with Serum Neurofilament Light Expression

摘要 目的评估酒精依赖患者脑白质病变(WML)的程度,监测血清神经丝轻链蛋白(NfL)的表达变化,并探讨酒精依赖患者脑WML程度与NfL及认知功能的相关性。方法将2018年9月至2020年12月郑州大学第一附属医院通过社会招募纳入的40例酒精依赖患者(酒精依赖组)和20例非饮酒者(对照组)作为研究对象。于门诊进行磁共振成像检查,通过Fazekas量表评定研究对象脑WML程度,通过单分子阵列检测酒精依赖组和对照组血清NfL表达变化,通过蒙特利尔认知评估量表(MoCA)评估酒精依赖患者的认知功能,分析脑WML程度与NfL水平和认知功能的相关性。结果酒精依赖患者Fazekas量表评分、NfL水平高于对照组,MoCA评分低于对照组,WML评分与NfL水平呈正相关,与MoCA评分呈负相关。结论酒精依赖患者脑WML较重,认知功能下降,NfL可能为评估酒精依赖脑白质损伤严重程度的生物标志物。 Objective To evaluate the severity of cerebral white matter lesion(WML)in patients with alcohol dependence,detect the expression of serum neurofilament light chain protein(NFL),and investigate the correlation of WML severity and NfL level in alcohol dependence patients.Methods From September 2018 to December 2020,40 patients with alcohol dependence(alcohol dependence group)and 20 non drinkers(control group)were recruited from the First Affiliated Hospital of Zhengzhou University.The brain WML was assessed by Fazekas scale.The expression of serum NFL in alcohol dependent group and control group was detected by single molecule array.The cognitive function of alcohol dependent patients was assessed by Montreal cognitive assessment scale(MoCA).The correlation between brain WML and NFL level and cognitive function was analyzed.Results Fazekas scale score and NFL level of alcohol dependence patients were higher than those of the control group,MoCA score was lower than that of control group,WML score was positively correlated with NFL level,and negatively correlated with MoCA score.Conclusion WML is more severe and cognitive function is decreased in alcohol dependent patients.NFL may be used as a biomarker to evaluate the severity of white matter injury in alcohol dependent patients.
