血清C反应蛋白、白细胞介素-6与乳腺癌患者术后骨转移的关系

The Relationship Between Serum C-Reactive Protein,Interleukin-6 Levels and Postoperative Bone Metastasis in Patients with Breast Cancer

目的探讨血清C反应蛋白(CRP)、白细胞介素(IL)-6与乳腺癌患者术后骨转移的关系。方法选取2018年1月至2019年12月郑州大学第二附属医院收治的70例接受乳腺癌改良根治术的乳腺癌患者作为研究对象,术后第3个月进行为期1 a的随访,根据患者术后骨转移情况,分为发生组和未发生组。比较两组术后1周血清CRP、IL-6水平,并分析血清CRP、IL-6水平与乳腺癌患者术后骨转移的关系。结果70例乳腺癌患者术后发生骨转移19例,占27.14%。两组年龄、体质量指数、病变部位、肿瘤直径、TNM分期比较,差异无统计学意义(P>0.05);发生组血清CRP、IL-6水平高于未发生组,差异有统计学意义(P<0.05)。经logistic回归分析,结果显示,血清CRP、IL-6水平高表达是乳腺癌患者术后骨转移的影响因素(OR>1,P<0.05)。绘制ROC曲线图,结果显示,血清CRP、IL-6水平分别预测乳腺癌术后骨转移的AUC>0.80,联合预测乳腺癌术后骨转移的AUC>0.85,当取最佳阈值时可获得最佳预测价值。结论血清CRP、IL-6水平高表达与乳腺癌患者术后发生骨转移密切相关。

Objective To investigate the relationship between serum C-reactive protein(CRP)and interleukin(IL)-6 levels and postoperative bone metastasis in patients with breast cancer.Methods Seventy cases of patients with breast cancer treated in the Second Affiliated Hospital of Zhenzhou University from January 2018 to December 2019 were prospectively selected as the research subjects,all patients underwent modified radical mastectomy,after operation 3 months,the patients were followed up for 1 year,according to the postoperative bone metastasis,the patients were divided into an occurrence group and a non metastasis group.The levels of serum CRP and IL-6 were compared between the two groups one week after operation,the relationship between serum CRP,IL-6 levels and postoperative bone metastasis of breast cancer patients was analyzed.Results Among 70 cases of patients with breast cancer,19 cases had bone metastasis,accounting for 27.14%;the serum levels of CRP and IL-6 in the occurrence group were higher than those in the non occurrence group,the difference was statistically significant(P<0.05);by logistic regression analysis,the results showed that high levels of serum CRP and IL-6 were the influencing factors of postoperative bone metastasis in breast cancer patients(OR>1,P<0.05);the ROC curve was drawn,and the results showed that the AUC of serum CRP and IL-6 levels in predicting bone metastasis after breast cancer surgery were>0.80,the AUC of combined prediction of bone metastasis after breast cancer surgery was>0.85,when the best threshold was chosen,the best predictive value could be obtained.Conclusion High levels of serum CRP and IL-6 are closely related to bone metastasis in breast cancer patients,the detection of serum CRP and IL-6 levels can be used in clinic to evaluate the postoperative bone metastasis of patients.
