阿替普酶静脉溶栓治疗急性脑梗死后脑出血转化的相关因素分析

Analysis of Related Factors in the Treatment of Cerebral Hemorrhage after Acute Cerebral Infarction by Intravenous Thrombolytic Therapy with Alteplase

目的关于阿替普酶静脉溶栓治疗方案对急性脑梗死后脑出血转化的相关因素调查。方法选择2018年1月至2019年4月来我院进行阿替普酶静脉溶栓治疗的急性脑梗死后脑出血转化的33例患者作为本文的观察组,另外选择同期来我院进行阿替普酶静脉溶栓治疗的急性脑梗死后未出现脑出血转化的33例患者作为对照组,对两组患者临床资料进行统计和综合的评价,分析和调查阿替普酶静脉溶栓治疗以后急性脑梗死后脑出血转化的相关因素。结果观察组当中合并高血压患者32例,合并糖尿病患者28例,合并心房颤动患者33例,存在卒中史患者27例,术前NIHSS评分≥21分者28例,溶栓之前血糖浓度≥6.1 mmol/L者30例,而对照组分别为2、4、4、5、3、3例,两组比较存在统计学差异,P<0.05。结论通过阿替普酶静脉溶栓对急性脑梗死患者进行治疗之后出现脑出血转化的原因表现在多个方面,所以临床要针对相关的情况进行有效的对症处理,以最大的程度上保证患者治疗的安全性。

Objective To investigate the related factors of intraperitoneal thrombolytic therapy in the treatment of cerebral hemorrhage after acute cerebral infarction.Methods From January 2018 to April 2019,33 patients with cerebral hemorrhage after acute cerebral infarction treated with intravenous thrombolytic therapy with alteplase were selected as the observation group of this study.33 patients with no cerebral hemorrhage after acute cerebral infarction treated with intravenous thrombolysis as a control group,statistical and comprehensive evaluation of clinical data of the two groups of patients,analysis and investigation of the early treatment of alteplase after intravenous thrombolytic therapy related factors of cerebral hemorrhage after acute cerebral infarction.Results Among the observation group,32 patients with hypertension,28 patients with diabetes,33 patients with atrial fibrillation,27 patients with history of stroke,28 patients with preoperative NIHSS score≥21 points,30 cases blood glucose concentration≥6.1 mmol/L before thrombolysis.In the contral group,there were 2 cases,4 cases,4 cases,5 cases,3 cases,and 3 cases,and there were statistical difference between the two groups(P<0.05).Conclusion The causes of cerebral hemorrhage after treatment of acute cerebral infarction by intravenous thrombolytic therapy with alteplase are manifested in many aspects.Therefore,it is necessary to effectively treat the relevant conditions in order to ensure the treatment of patients to the greatest extent.Security.
