新诊断2型糖尿病患者血清鸢尾素水平与腹腔内脏脂肪面积关系的研究

Relationship between serum Irisin and abdominal visceral fat area in patients with newly diagnosed type 2 diabetes mellitus

目的探讨新诊断T2DM患者血清鸢尾素(Irisin)水平与腹腔内脏脂肪面积(VFA)的关系。方法选取2016年5月至2018年12月于联勤保障部队第九〇三医院内分泌科门诊及住院患者211例,根据OGTT结果分为新诊断T2DM组(n=106)、糖耐量正常组(NGT,n=105)。收集两组患者基线资料,测定生化指标及血清Irisin水平,计算机断层扫描测定腹腔VFA与皮下脂肪面积(SFA)。结果与NGT组比较,T2DM组WC、WHR、DBP、FPG、HbA1c、TC、TG、HOMA-IR、FFA、VFA、VFA/SFA、VFA%升高(P<0.01),胰岛β细胞功能、HDL-C、Irisin降低(P<0.05)。Pearson相关分析显示,Irisin与体重、WC、WHR、TC、LDL-C、VFA呈正相关(r=0.208、0.239、0.196、0.382、0.463、0.309,P<0.05或P<0.01)。Spearman相关分析显示,Irisin与HbA1c、TG、VFA/SFA呈正相关(r=0.203、0.196、0.243,P<0.05)。多元线性回归分析显示,TC、LDL-C、VFA是血清Irisin水平的影响因素。结论新诊断T2DM患者血清Irisin水平较正常人群降低,与VFA呈正相关。

Objective To investigate the association between serum Irisin level with abdominal visceral fat area(VFA)in patients with newly diagnosed type 2 diabetes mellitus(T2 DM).Methods A total of 211 patients were enrolled in this study from the outpatient clinic and ward in the Endocrinology Department in NO. 903 Hospital of PLA Joint Logistic Support Force from May 2016 to December 2018. All the patients were divided into two groups according to OGTT results:newly diagnosed T2 DM group(n=106)and normal glucose tolerance group(NGT,n=105). Biochemical indexes were measured in the two groups.Serum Irisin level was measured by ELISA,and abdominal VFA and subcutaneous fat area(SFA)were measured by computed tomography(CT).Results Compared with the NGT group,the WC,WHR,DBP,FPG,HbA1c,TC,TG,HOMA-IR,FFA,VFA,VFA/SFA and VFA% were increased(P<0. 01),while the contents of HOMA-β,HDL-C and Irisin were decreased(P<0. 05)in the T2 DM group. Pearson correlation analysis showed that circulating Irisin was positively correlated with body weight,WC,WHR,TC,LDLC,and VFA(r=0. 208,0. 239,0. 196,0. 382,0. 463,0. 309,P<0. 05 or P<0. 01). Spearman correlation analysis showed that serum Irisin level was positively correlated with HbA1c,TG and VFA/SFA(r=0. 203,0. 196,0. 243,P<0. 05). Multivariable linear regression analysis revealed that TC,LDL-C and VFA were the influencing factors for serum Irisin level.Conclusion Serum Irisin level was decreased in patients with newly diagnosed type 2 diabetes mellitus,and positively associated with visceral fat area.
