综合医院16573例针灸会诊病例适应证及学科分布回顾性分析

Indications and Discipline Distribution Characteristics of 16573 Cases with Acupuncture and Moxibustion Consultation in General Hospitals: A Retrospective Study

目的探讨针灸在综合医院中的适应证与应用特色。方法收集2015年1月1日至2019年12月31日山东大学齐鲁医院针灸推拿科针灸会诊的16573例住院患者的临床资料,统计会诊科室来源、会诊病症、学科分布、疾病所属系统等。结果申请针灸会诊的科室共33个;会诊病症分布达153种,主要集中在神经系统疾病、脑血管疾病、术后并发症、消化系统疾病及关节肌肉疾病等,会诊最多的前4个病症依次为脑梗死后遗症(4082例)、腹胀(1750例)、胃肠功能障碍(1083例)、脑出血后遗症(715例);会诊病例共涉及17个二级学科,排名前4位的依次为外科学(5379例)、神经病学(4666例)、妇产科学(1612例)、内科学(1436例);会诊病症的原发疾病涉及19个系统,排名前4位的依次为循环系统(5543例),肿瘤(3673例),神经系统(1709例),肌肉骨骼系统和结缔组织疾病(1487例)。结论针灸治疗在综合医院多个科室应用广泛,适应症主要集中在脑血管疾病、手术并发症、消化系统疾病及关节肌肉疾病等方面,在手术并发症、放化疗不良反应以及药物毒副反应的防治方面颇具特色。

Objective To explore the indications and application characteristics of acupuncture and moxibustion in general hospitals. Methods The clinical data of 16573 inpatients who obtained acupuncture and moxibustion consultations in the Acupuncture, Moxibustion and Tuina Department of Qilu Hospital of Shandong University from January 1 st, 2015 to December 31 st, 2019 were collected and analyzed, including the source department, the consultation disease, the disciplines distribution and the disease system. Results A total of 33 departments applied for consultation of acupuncture and moxibustion, involving 153 types of diseases, which were mainly nervous system diseases, cerebrovascular diseases, postoperative complications, digestive system diseases, joint and muscle diseases;the top four diseases for consultation of acupuncture and moxibustion were sequelae of cerebral infarction(4082 cases), abdominal distension(1750 cases), gastrointestinal dysfunction(1083 cases), sequelae of cerebral hemorrhage(715 cases);17 secondary disciplines were involved, of which surgery(5379 cases), neurology(4666 cases), obstetrics and gynecology(1612 cases) and internal medicine(1436 cases) were mostly seen. The primary diseases could be classified into 19 disease systems, and the top four systems with the highest proportion were circulatory system(5543 cases), tumors(3673 cases), nervous system(1709 cases), musculoskeletal system and connective tissue diseases(1487 cases). Conclusion Acupuncture and moxibustion treatment is widely used in multiple departments of general hospitals, and the indications are mainly cerebrovascular diseases, surgical complications, digestive system diseases, joint and muscle diseases. It also plays a quite unique role in the prevention and treatment of surgical complications, adverse reactions of radiotherapy and chemotherapy, and drug toxicity.
