小骨瓣开颅内镜辅助下手术治疗老年急性硬膜下血肿的疗效观察

Small bone flap craniotomy assisted by endoscopic surgery in treatment of acute subdural hematoma in elderly

目的探讨小骨瓣开颅内镜辅助下手术治疗老年急性硬膜下血肿(ASDH)的安全性及有效性。方法回顾性分析19例老年创伤性急性硬膜下血肿应用小骨瓣开颅内镜辅助下手术清除血肿患者的临床资料;对患者的影像学特点、手术方式及预后进行分析。结果根据格拉斯哥预后量表(GOS)评分评定预后,本组患者中恢复良好15例、轻度残疾3例、死亡1例(因心肺并发症致死)。结论应用小骨瓣开颅内镜辅助下清除老年急性硬膜下血肿手术方式安全、有效,手术并发症少,值得临床推广使用。

Objective To investigate the safety and effectiveness of small bone flap craniotomy assisted by endoscope in the treatment of elderly patients with acute subdural hematoma(ASDH).Methods The clinical data of 19 elderly patients with traumatic acute subdural hematoma treated with small bone flap craniotomy assisted by endoscope were analyzed retrospectively.The imaging characteristics,surgical data and prognosis were analyzed.Result According to Glasgow outcome score(GOS),15 cases recovered well,3 had mild disability,and one died of cardiopulmonary complications.Conclusion Small bone flap craniotomy assisted by endoscopy is safe and effective in the treatment of elderly patients with ASDH.
