加味圣愈汤治疗分娩会阴深Ⅱ度裂伤效果及对创面愈合及促炎因子的影响

Effect of Jiawei Shengyu Decoction(加味圣愈汤)on Wound Healing and Proinflammatory Factors in Maternal Depression

目的探讨加味圣愈汤治疗阴道分娩会阴深Ⅱ度裂伤产妇的临床应用价值。方法选取医院2018年1月—2019年12月阴道分娩导致会阴深Ⅱ度裂伤产妇200例,采用随机数字表法分为A组和B组各100例,两组均予以常规缝合、术后创面消毒等基础治疗措施,A组产妇加服加味圣愈汤连续5 d,对比两组产妇产后会阴疼痛、创面愈合时间及级别、血清白细胞介素-1(IL-1)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)的变化。结果产后第1天,两组产妇的VAS评分差异不具有统计学意义(P>0.05);产后第3天、第5天,B组产妇的VAS评分高于A组(P<0.05);A组产妇会阴切口愈合时间显著的低于B组(P<0.05),A组产妇会阴切口愈合级别优于B组(P<0.05);产后第1天,两组产妇的血清IL-1、IL-6、IL-10、TNF-α水平差异不具有统计学意义(P>0.05);产后第3天,A组产妇的血清IL-1、IL-6、IL-10、TNF-α水平均低于B组(P<0.05)。结论加味圣愈汤治疗阴道分娩会阴深Ⅱ度裂伤产妇具有降低炎症反应程度、促进创面愈合的作用。

Objective To explore the clinical application value of Jiawei Shengyu Decoction(加味圣愈汤) in the treatment of vaginal delivery in patients with deep and second degree laceration. Methods A total of 200 cases of vaginal delivery from January 2018 to December 2019 resulted in perineal deep II degree laceration, were divided into group A and group B by random number table. Basic treatment measures such as suturing and postoperative wound disinfection were used. Group A took Jiawei Shengyu Decoction for 5 consecutive days. The postpartum perineal pain, wound healing time and grade, serum interleukin-1(IL-1) and leukocyte mediation were compared between the two groups. Changes in interleukin-6(IL-6), interleukin-10(IL-10) and tumor necrosis factor-α(TNF-α) were observed. Results On the 1 st day after delivery, the difference of VAS scores between the two groups was not statistically significant(P>0.05). On the 3 rd and 5 th day after delivery, the VAS scores of the women in group B were higher than those in group A(P<0.05). The healing time was significantly lower than that of group B(P<0.05). The level of maternal perineal incision healing in group A was better than that in group B(P<0.05). On the first day after delivery, serum levels of IL-1, IL-6,IL-10 and TNF-α were not statistically significant(P>0.05). On the 3 rd day after delivery, the serum levels of IL-1, IL-6, IL-10 and TNF-α in group A were lower than those in group B(P<0.05). Conclusion Jiawei Shengyu Decoction for the treatment of vaginal delivery and deep second degree laceration of the vaginal delivery has the effect of reducing the degree of inflammatory reaction and promoting wound healing.
