头针配合MOTOmed对痉挛型脑瘫下肢功能的影响

Effect of Scalp Acupuncture Combined with MOTOmed on Lower Limb Function of Patients with Spastic Cerebral Palsy

目的探讨头部针刺配合MOTOmed训练对痉挛型脑瘫患者下肢功能的影响及其机制。方法选取2017年7月—2019年7月收治的痉挛型脑瘫儿116例作为研究对象,根据干预措施分为试验组和对照组,每组58例。对照组进行单独MOTOmed训练,试验组在对照组治疗的基础上,做头部针刺辅助治疗。于治疗前和治疗10周后,用改良巴氏指数评定表(PADL)和粗大运动功能(GMFM)系统评分对2组患者治疗前后下肢功能进行评定,用ELISA方法分别测定血清中NGF、ET含量。结果2组治疗前GMFM、PADL分值以及NGF、ET差异无统计学意义(P>0.05);治疗后10周后试验组GMFM、PADL分值以及NGF水平明显提高(P<0.05),而ET水平治疗前后差异无统计学意义(P>0.05)。结论头部针刺配合进行MOTOmed训练模式对脑瘫儿下肢功能障碍有明显的改善效果,其机制与血清中NGF水平有关。

Objective To investigate the effect and mechanism of scalp acupuncture combined with MOTOmed training on lower limb function in patients with spastic cerebral palsy.Methods A total of 116 children with spastic cerebral palsy admitted to our hospital from July 2017 to July 2019 were selected as objects,and were divided into experimental group and control group according to intervention measures,with 58 cases in each group.The control group received MOTOmed training alone,while the experimental group received scalp acupuncture and moxibustion therapy on the basis of treatment in the control group.Before and 10 weeks after treatment,the lower extremity functions of the two groups were evaluated by PADL and GMFM scores,and the levels of NGF and ET in serum were determined by ELISA.Results There was no significant difference in GMFM,PADL scores,NGF and ET between the two groups before treatment(P>0.05).After 10 weeks of treatment,GMFM,PADL scores and NGF levels in the experimental group were significantly increased(P<0.05),while there was no statistical difference in ET levels before and after treatment(P>0.05).Conclusion MOTOmed training combined with scalp acupuncture has an obvious effect on the improvement of lower limb dysfunction in children with cerebral palsy,and its mechanism is related to the level of NGF in serum.
