处方点评在抗菌药物临床干预中的应用效果及对抗菌药物使用率的影响分析

Prescription reviews in the clinical intervention of antibacterial drugs and ana lysis of their influence on the use rate

目的研究处方点评在抗菌药物临床干预中的应用效果及对抗菌药物使用率的影响.方法回顾性收集我院门急诊2019年1月~2020年12月的药物处方8000份,其中2019年1月~12月的药物处方4000份为对照组,未给予处方点评;2020年1月~12月的药物处方4000份为干预组,给予处方点评,整理两组的抗菌药物使用率以及抗菌药物的使用合理率等.结果干预组的抗菌药物使用率15.125%低于对照组的29.775%,P<0.05;且在抗菌药物使用处方中:对照组的抗菌药物使用不合理率14.19%高于干预组的7.93%,P<0.05;在抗菌药物使用不合理的原因上:两组在用法用量不合理、溶媒用量不合理和重复用药的概率差异不明显,P>0.05;但在选择药物不合理、配伍不合理、无指征使用的概率方面干预组均低于对照组,P<0.05;抗菌药物使用处方中,干预组患者的不良反应发生率低于对照组,P<0.05.结论处方点评的开展有助于提高门急诊的抗菌药物使用合理性水平,减少药物不良反应,保证抗菌药物使用的安全性,但是依然需要进一步加强对抗菌药物使用的管理.

Objective To study the effect of prescription reviews in clinical intervention of antibacterial drugs and its influence on the use rate of antibacterial drugs.Methods A retrospective collection of 4000 drug prescriptions from January 2019 to December 2020 in the outpatient and emergency department of our hospital,of which 4000 prescriptions from January to December 2019 were used as the control group,and no prescription reviews were given;January 20204000 prescriptions of drugs in~12 months were the intervention group,and the prescriptions were reviewed,and the antibacterial drug use rate of the two groups and the reasonable rate of antibacterial drug use were sorted out.Results 15.125%of the antimicrobial use rate in the intervention group was lower than 29.775%in the control group,<0.05;and in the prescription of antimicrobial drugs:the unreasonable rate of 14.19%antimicrobial use in the control group was higher than that in the intervention group The reason for the unreasonable use of antibacterial drugs:the two groups have no significant difference in the usage and dosage,the solvent dosage,and the probability of repeated medication,P>0.05;but the choice of drugs is unreasonable,The probability of unreasonable compatibility and no indication in the intervention group was lower than that in the control group,P<0.05;in the prescription of antibiotics,the incidence of adverse reactions in the intervention group was lower than that in the control group,P<0.05.Conelusion The development of prescription reviews can help improve the rationality of antimicrobial use in outpatient and emergency departments,reduce adverse drug reactions,and ensure the safety of antimicrobial use.However,there is still a need to further strengthen the management of antimicrobial use.
