中青年痛风患者血尿酸水平与代谢指标变化趋势的相关性研究

Study on the correlation between the level of serum uric acid and metabolic index in young and middle-aged patients with gout

目的探讨中青年痛风患者血尿酸(UA)水平与代谢指标变化趋势的相关性及其临床意义.方法选择2019年6月~2020年6月在我院诊治的中青年痛风患者150例,其中高尿酸血症(HUA)患者114例,血尿酸(NUA)正常患者36例,同时选择150名体检健康者为对照组,所有研究对象测量身高、体重及血压,并检测血清甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、空腹血糖(FBG)及UA水平.结果痛风组体重指数(BMI)、收缩压(SBP)、舒张压(DBP)、TG、LDL-C、FBG及UA水平均显著高于对照组(t=5.231,7.090,11.137,8.550,9.776,5.618,29.809,P均<0.05).HUA组BMI、SBP、TG、LDL-C、FBG及UA水平均显著高于NUA组(t=3.913,3.491,3.749,4.852,2.759,17.876,P均<0.05).多因素Logistic回归结果显示,BMI、TG、LDL-C及FBG为UA水平升高的危险因素(OR=2.248,1.375,8.492,5.375,P均<0.05).相关性分析结果显示,痛风患者血UA水平与BMI、SBP、TG、LDL-C及FBG均呈正相关(r=0.549,0.372,0.440,0.632,0.432,P均<0.05).结论中青年痛风患者血UA升高与BMI、血压、高血脂、血糖等代谢指标密切相关,上述指标可能相互作用、相互影响,共同促进了痛风的发生和发展.通过观察UA和其他代谢指标变化趋势可为痛风的监测和干预提供参考.

Objective To investigate the correlation between the level of serum uric acid(UA)and metabolic indexes and its clinical significance in young and midrlle-aged patients with gout.Methods Totally 150 young and middle-agerl patients with gout in our hospital were selected from June 2019 to June 2020,there were 114 patients with hyperuricemia(HUA)and 36 patients with normal level of blood uric*arid(NUA).At the same time,150 healthy people with meciiral examination were selected as the control group.Height,weight and blood pressure were measured and the levels of serum triglyceride(TG),total cholesterol(TC),lovv density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),fasting blood-glucose(FBG)and UA were detected in all suhjects.Resu I ts The levels of body mass index(BMI),systolic blood pressure(SBP).rliaslolio blood pressure(DBP),TG,LDL-C,FBG and UA in gout group were significantly higher than those in ronlrol group(t=5.231,7.090,11.137,8.550,9.776,5.618,29.809,all P<0.05).The levels of BMI,SBP,TG,LDL-C,FBG and UA in HUA group were significantly higher than those in NUA group(t=3.913,3-491,3-749,4.852,2.759,17.876,all P<0.05).The results of multivariate logistic regression showed that BMI,TG,LDL-C and FBG were the risk factors for the increase of the serum level of UA(0/?=2.248,1.375,8.492,5.375,allP<0.05).The results of correlation analysis showed lhal the serum UA was positively correlated with BMI,SBP,TG,LDL-C and FBG in patients with gout(r-=0.549,0.372,0.440,0.632,0.432,311P<0.05).Conc lusions The increase of serum UA in young and middle-aged patients with gout is closely related to the metabolic indexes surh as BMI,bloorl pressure,hyperlipidemia and blood glucose,which maybe interarl and influence each other and promote the occurrence and development of gout together.Therefore,observing the trends of UA and other metabolic indicators ran provide reference for monitoring and intervening of gout.
