新生儿呼吸窘迫综合征超声诊断与分度专家共识 被引量:31

Expert consensus on ultrasound diagnosis and gradation of neonatal respiratory distress syndrome

摘要 新生儿肺脏疾病超声诊断技术日益成熟,尤其对新生儿呼吸窘迫综合征(respiratory distress syndrome,RDS)的诊断具有高度敏感性和特异性;在肺脏超声(lung ultrasound,LUS)监测指导下管理患儿,极大地改善了RDS预后。为了有助于LUS的开展和应用,更好地发挥LUS在RDS管理中的优势,特组织有关专家拟定了本专家共识,对超声仪器的选择、参数调节、基本操作方法、RDS的超声诊断及分度标准等进行了详尽介绍。专家共识认为肺实变是RDS最重要的超声影像学特点,是LUS诊断RDS的必备条件。根据肺实变程度和范围及是否导致了严重并发症,将RDS分为轻、中、重三度,从而优化对RDS的管理。 Using ultrasound to diagnose lung diseases has emerged and been becoming popular,especially for the diagnosis of neonatal respiratory distress syndrome(RDS),which has higher sensitivity and specificity.The prognosis of RDS patients has been greatly improved under the monitoring of lung ultrasound(LUS).To improve the application of LUS in the management of RDS patients,we organized the expert panel to develop this consensus.The selection of ultrasound equipment,parameter adjustment,operation skills,RDS ultrasound diagnostic standards as well as the grading standards were introduced in details.It is believed that lung consolidation is the most important image feature of RDS,which is essential for diagnosis of RDS through LUS.RDS can be divided into three degrees,which are mild(GradeⅠ),moderate(GradeⅡ),and severe degree(GradeⅢ)of RDS according to the lung consolidation conditions and whether it has resulted in severe complications,which is benefical to the management of RDS patients.
