术前个体化饮食干预对择期妊娠糖尿病产妇剖宫产术中血糖水平的影响

The effect of preoperative individualized dietary intervention on blood sugar level of parturients with gestational diabetes mellitus undergoing cesarean section

目的探讨给予择期剖宫产妊娠糖尿病(GDM)患者术前个体化饮食干预的价值及对患者血糖水平的影响。方法选取2018年6—12月在某院接受孕检并择期行剖宫产的GDM患者100例,分为两组,常规组(n=50)术前行常规饮食护理,干预组(n=50)再加强术前个体化饮食护理,观察患者血糖控制、妊娠结局等情况。结果干预组产妇术中、术后血糖水平相比常规组均显著更高;干预组新生儿血糖水平显著高于常规组;干预组产妇酮症发生率均显著低于常规组,低血糖、肺透明膜病(呼吸窘迫综合征)等围产儿并发症发生率均显著低于常规组,差异均有统计学意义(P<0.05)。结论给予择期行剖宫产GDM早期术前个体化饮食干预,可有效提高血糖控制效果,降低产妇、围产儿并发症发生风险。

Objective To evaluate the value of individualized dietary intervention before elective cesarean section for gestational diabetes mellitus(GDM)patients and its effect on blood sugar level.Methods 100 GDM patients who underwent pregnancy examination and elective cesarean section in our hospital from June to December 2018 were divided into two groups,routine nursing intervention was performed in the routine group(n=50)before operation,and individualized diet nursing was strengthened in the intervention group(n=50),blood sugar control,changes of vital signs during operation and pregnancy outcome were observed.Results The blood sugar level of intervention group was significantly higher than that of routine group during and after operation;Neonatal blood sugar level in intervention group was significantly higher than that in routine group;The incidence of maternal ketosis in intervention group was significantly lower than that in routine group,and the incidence of perinatal complications such as hypoglycemia,respiratory distress syndrome and growth restriction was significantly lower than that in routine group,the differences were statistically signifiant(P<0.05).Conclusion Individualized dietary intervention before early GDM of cesarean section was given,can effectively improve the effect of blood sugar control,can effectively improve the effect of blood sugar control and reduce the risk of complications of maternal and perinatal infants.
