功能训练联合针刺对脑瘫患儿吞咽功能障碍的疗效观察

Observation of the effect of functionaltraining combined with acupuncture on the swallowing dysfunction in children with cerebral palsy

目的观察常规功能训练联合针刺改善脑瘫患儿吞咽功能障碍的临床疗效。方法选择2016年3月至2018年12月期间在陕西中医药大学附属西安中医脑病医院收治住院的89例脑瘫伴有吞咽功能障碍的210岁患儿为研究对象,随机分为治疗组(44例)和对照组(45例)。对两组患儿均进行基础康复训练和吞咽功能训练,治疗组增加针刺治疗,每个疗程为4周,连续治疗2个疗程后,采用改良洼田饮水试验测评其吞咽功能。结果治疗2个疗程后,治疗组和对照组的改良洼田饮水试验测评值均显著低于治疗前(t值分别为8.41、4.38,P<0.05),且治疗组的改良洼田饮水试验测评值显著低于对照组(t=3.54,P<0.05);治疗组的治疗有效率为90.9%(40/44),显著高于对照组的64.4%(29/45),差异有统计学意义(χ^(2)=12.99,P<0.05)。结论常规功能训练联合针刺改善脑瘫患儿吞咽功能的疗效明显,值得临床推广应用。

Objective To observe the clinical effect of routine functional training combined with acupuncture on improving the swallowing dysfunction in children with cerebral palsy.Methods A total of 89 children aged 2~10 years with cerebral palsy and swallowing dysfunction,who were hospitalized in the Affiliated Xi′an Traditional Chinese Medicine Brain Disease Hospital of Shaanxi University of Traditional Chinese Medicine between March 2016 and December 2018,were selected as the study partici-pants.They were divided into the treatment group(44 cases) and the control group(45 cases) randomly.All children of the two groups were treated with basic rehabilitation training and swallowing function training,and acupuncture treatment was added in the treatment group.Each course was 4 weeks,and after continuous treatment of two courses,the modified Kubota′s water swallowing test was used to evaluate the swallowing function.Results After treatment of 2 courses,the modified Kubota′s water swallowing test score of both the treatment group and control group were significantly lower than those before treatment(t=8.41 and 4.38,respectively,both P<0.05),and the score of the treatment group was significantly lower than that of the control group(t=3.54,P<0.05).The effective rate of the treatment group was 90.9%(40/44),which was significantly higher than 64.4%(29/45) of the control group(χ^(2)=12.99,P<0.05).Conclusion Routine functional training combined with acupuncture has obvious curative effect on improving the swallowing function in children with cerebral palsy,and it is worthy of clinical application.
