妊娠期胃癌3例病例报道并文献复习

Case report and literature review of 3 cases of gastric cancer during pregnancy

目的探讨妊娠期胃癌的临床特点,加强对该类疾病的认识,改善其不良预后。方法查阅相关文献,回顾2018年至今,空军军医大学第一附属医院产科收治的3例妊娠期胃癌患者的临床资料,并对患者进行随访。结果3例妊娠期胃癌患者的年龄为2628岁,临床表现为恶心、呕吐、上腹部疼痛。1例发现盆腔包块,晚孕期行剖宫产术终止妊娠,早产儿评分可,存活,胃镜活检及病理检查证实为胃癌;2例行中孕期引产术终止妊娠。结论对有适应症的患者应积极行胃镜检查,早期检查,早期明确诊断,根据孕周、胃癌分期及孕妇的意愿,由多学科团队实施个体化治疗,包括手术或化疗,是改善妊娠期胃癌患者不良预后的有效方法。

Objective To investigate the clinical features of gastric cancer during pregnancy,strengthen the understanding of this kind of disease,and improve its poor prognosis.Methods Through searching and reviewing related literature,the clinical data of 3 pregnant women with gastric cancer who were admitted to department of obstetrics,the First Affiliated Hospital of Air Force Military Medical University from 2018 to now were reviewed and the patients were followed-up.Results The age of 3 pregnant women with gastric cancer were 26~28 years old,and the clinical symptoms included nausea,vomiting,and epigastric pain.1 case found a pelvic mass,and cesarean section was adopted in the third trimester to terminate the pregnancy.The preterm infant was live and the score was eligible.The pelvic mass was confirmed as gastric cancer by gastroscopy biopsy and pathological examination.2 cases underwent induction of labor in the second trimester to terminate the pregnancy.Conclusion Patients with indications should be positively treated with gastroscopy,early examination and early definite diagnosis,according to gestational age,stage of gastric cancer,and willing of the pregnant women,individual tailored treatment was conducted by the multi-disciplinary team,containing operation and chemotherapy,which were the effective ways to improve the poor prognosis of patients with gastric cancer during pregnancy.
