超声心动图联合浅表高频超声在肺动脉瓣狭窄患儿瓣膜数诊断中的临床价值及误诊率分析

Clinical value and misdiagnosis rate of echocardiography combined with superficial high frequency ultrasound in the diagnosis of valve number in children with pulmonary stenosis

摘要 目的探讨在肺动脉瓣狭窄患儿瓣膜数诊断中采用超声心动图检查联合浅表高频超声检查的临床价值。方法选取2020年1月~2020年12月阳谷县人民医院收治的30例疑似肺动脉瓣狭窄患儿的临床资料,所有患儿均进行超声心动图检查、浅表高频超声检查以及二者联合检查。跟访到手术病理确诊,并以此为标准,对三种检查诊断结果进行比较,分析其检查准确性和误诊情况。结果手术病理结果显示,13例患儿瓣膜数目正常,17例患儿瓣膜数目异常。超声心动图诊断显示,12例患儿瓣膜数目异常,18例患儿瓣膜数目正常;浅表高频超声诊断显示,13例患儿瓣膜数目异常,17例患儿瓣膜数目正常;联合诊断显示,16例患儿瓣膜数目异常,14例患儿瓣膜数目正常;联合诊断的敏感度为94.12%,相较于超声心动图的70.59%、浅表高频超声的76.47%均更高,差异均有统计学意义(P<0.05)。联合诊断的误诊或漏诊率为5.88%,相较于超声心动图的29.41%、浅表高频超声的23.53%均更低,差异均有统计学意义(P<0.05)。结论在肺动脉瓣狭窄患儿瓣膜数诊断中采用超声心动图检查联合浅表高频超声检查进行临床诊断,特异度和敏感度较高,可获得比较清晰的患儿肺动脉瓣瓣膜的二维图像,更加准确地诊断出患儿的瓣膜数目,减少误诊率,为患儿的临床诊断与治疗提供有效依据。 Objective To explore the clinical value of echocardiography combined with superficial high frequency ultrasound in the diagnosis of valve number in children with pulmonary stenosis.Methods During the period from January 2020 to December 2020,30 cases of suspected pulmonary valve stenosis were treated in our hospital.All cases were examined by echocardiography,superficial high-frequency ultrasound and the combination of the two.The diagnosis was confirmed by surgery and pathology.Based on this,the diagnostic results of the three methods were compared,and the accuracy and misdiagnosis of the three methods were analyzed.Results Surgical pathology results:13 cases of children with normal valve number,17 cases of children with abnormal valve number.Echocardiography showed that the number of valves was abnormal in 12 cases and normal in 18 cases;superficial high-frequency ultrasound showed that the number of valves was abnormal in 13 cases and normal in 17 cases;combined diagnosis showed that the number of valves was abnormal in 16 cases and normal in 14 cases;the sensitivity of combined diagnosis was 94.12%,compared with 70.59%of echocardiography The misdiagnosis or missed diagnosis rate of combined diagnosis was 5.88%,which was significantly lower than that of echocardiography(29.41%)and superficial high frequency ultrasound(23.53%)(P<0.05).Conclusion In the diagnosis of pulmonary valve stenosis in children with valve number,echocardiography combined with superficial high-frequency ultrasound for clinical diagnosis has high specificity and sensitivity,which can obtain clear two-dimensional images of pulmonary valve in children,more accurately diagnose the number of valves in children,reduce the misdiagnosis rate,and provide effective basis for clinical diagnosis and treatment of children.
