肌间隙入路微创置入钉棒系统治疗胸腰椎骨折的疗效观察

摘要 目的探讨胸腰椎骨折采用肌间隙入路微创置入钉棒系统的疗效.方法选择2016年1月—2019年12月收治的胸腰椎骨折患者62例,分为传统入路组与肌间隙入路组各31例.传统入路组沿棘突两旁分离骶脊肌群分离进入;肌间隙入路组在棘突中线旁开2 cm纵行将两侧胸腰筋膜切开,自竖脊肌中钝性分离最长肌及多裂肌之间的间隙分离进入.比较2组围术期情况及近远期疗效.结果肌间隙入路组手术时间、术中出血量、术后引流量明显低于传统入路组(P<0.05).2组术后3 d视觉模拟评分法(VAS)评分无明显差异(P>0.05),肌间隙入路组术后个月、3个月明显低于传统入路组(P<0.05).2组术后椎体矢状面指数(VSI)指数、功能障碍指数、骨折愈合时间无明显差异(P>0.05).结论与传统入路相比,胸腰椎骨折采用肌间隙入路微创置入钉棒系统治疗手术时间短、创伤小、远期腰背痛发生率低.
