Wagner SL柄与TM杯治疗高龄转子间骨折伴关节炎的临床研究

Wagner SL Femoral stem combined with TM cup in the treatment of elderly patients with intertrochanteric fracture and hip osteoarthritis

目的探讨Wagner SL柄结合TM骨小梁杯治疗高龄股骨转子间骨折合并髋骨性关节炎的短期疗效观察。方法对10例骨折前有髋骨性关节炎的转子间骨折使用Wagner SL柄结合TM骨小梁杯进行全髋关节置换治疗,术后进行临床随访,采取Harris评分对患髋关节功能进行评价。影像学随访主要包括对假体周围骨长入,假体周围骨折愈合等进行评价。结果术后随访,时间4~26月,平均15月,术后3个月X线片提示10例患者的转子间骨折愈合良好。复诊时对患髋关节进行评价,根据Harris评分法:优5例,良3,可2例。结论Wagner SL柄结合TM骨小梁杯治疗合并髋骨性关节炎高龄股骨转子间骨折的患者可取得较好的临床疗效以及可靠的大小转子的重建,有利于假体稳定及关节活动,可增加假体周围骨量储备。

Objective To investigate the short-term efficacy of Wagner SL Femoral stem combined with TM trabecular cup in the treatment of elderly patients with intertrochanteric fractures complicated with osteoarthritis.Methods 10 cases of intertrochanteric fractures with hip osteoarthritis before fracture were treated Wagner SL Femoral stem and TM trabecular cup for total hip arthroplasty and sturdy.There were 4 males and 6 female patients,whose age ranged from 81 to 88 years,with an average of 84 years.Clinical follow-up was performed after surgery.The Harris score was used to evaluate the function of hip joint activity.Imaging follow-up mainly included evaluation of bone growth around the prosthesis and loosening of the prosthesis.Results Postoperative follow-up was carried out in the period from 4 to 26 months,with an average of 15 months.Three months after surgery,x-ray films showed that 10 patients with intertrochanteric fractures healed well.The patients were evaluated according to the Harris score:excellent in 5 cases,good in 3,and fair in 2 cases.Conclusion Wagner SL Femoral stem combined with TM trabecular cup can improve clinical efficacy and reconstruction of large and small femoral trochanter in the treatment of patients with femoral intertrochanteric fractures with hip osteoarthritis,which can also enhance the stability of the prosthesis and joint activities,and increase the bone reserve around the prosthesis.
