SchatzkerⅡ型胫骨平台骨折塌陷增宽和半月板损伤关系

The Relationship between collapse widening of lateral plateau and meniscus injury in Schatzker typeⅡtibial plateau fracture

目的通过X片对SchatzkerⅡ型胫骨平台骨折的外侧平台塌陷和增宽程度的测量,探讨其与半月板损伤的关系。方法从2015年12月至2018年12月,收集30例SchatzkerⅡ型胫骨平台骨折,测量前后位X片外侧平台的塌陷和增宽程度。所有患者均采取手术治疗,术中探查并明确半月板损伤情况,发现18例半月板损伤(损伤组),12例半月板未损伤(非损伤组)。然后比较两组的外侧平台塌陷和增宽差别是否具有统计学意义。结果损伤组:外侧平台塌陷压缩的平均值在18mm(IQR(四分位数的间距):12~24),平均外侧平台增宽为10mm(IQR(四分位数的间距):8~12)。非损伤组:相应的值为9mm(IQR(四分位数的间距):3~15),5mm(IQR(四分位数的间距):2~9)(P=0.001),差别具有统计学意义(P=0.001;P=0.001)。结论在前后位X片上,外侧塌陷超过12mm或者增宽超过8mm,半月板损伤的风险将显著性增高。

Objective To explore the collapse and widening of the tibial plateau fracture are the signs of meniscal injuries in Schatzker typeⅡtibial plateau fractures.Methods The lateral plateau collapse and lateral plateau widening were measured on anteroposterior knee radiographs in 30 patients with Schatzker typeⅡtibial plateau fracture from December 2015 to December 2018.All cases were treated by operation,and the meniscal injury was checked.18 patients with meniscal injury(five,meniscal tears.thirteen,peripheral meniscal detachments)were divided into injury group.The others were divided into noninjury group.The lateral plateau collapse and lateral plateau widening measurements were compared between two groups.Results In injury group,the median lateral plateau depression was 18mm(IQR:12-24)and the median lateral plateau widening was 10 mm(IQR:8-12).In noninjury group,the respective values were 9 mm(IQR:3-15)and 5 mm(IQR:2-9).There was a statistically significant difference in both parameters(P=0.001,P=0.001).Conclusion Lateral collapse more than 12 mm or widening more than 8 mm are related with the higher risk of meniscus injury.
