针灸辅助治疗消化性溃疡的疗效及血清TNF-α、IL-8、GAS的水平变化

The effect of acupuncture and moxibustion on peptic ulcer and the changes of serum TNF-α,IL-8 and gastrin

目的观察针灸辅助治疗消化性溃疡的疗效及血清肿瘤坏死因子α(TNF-α)、白细胞介素8(IL-8)、胃泌素(GAS)的水平变化。方法选取2018年6月至2020年6月于该院接受治疗的101例消化性溃疡患者为研究对象,根据随机数字表法分为观察组(n=51)与对照组(n=50)。2组均给予基础治疗,对照组加以常规西药治疗,观察组在对照组基础上实施针灸辅助治疗。比较2组短期(4周内)临床疗效,并记录治疗前、治疗4周后的炎症因子[TNF-α、IL-8、热休克蛋白60(HSP60)]、血清胃肠功能指标[GAS、生长抑素(SS)、胃动素(MTL)]、外周血T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))的变化及不良反应发生情况。结果治疗4周后,观察组治疗总有效率(94.12%)明显高于对照组(80.00%),差异有统计学意义(P<0.05)。治疗4周后,2组患者IL-8、TNF-α、HSP60、GAS、MTL、CD8^(+)较治疗前均显著下降,而SS、CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)较治疗前均显著升高,且观察组变化幅度更大,差异均有统计学意义(P<0.05)。治疗过程中,观察组不良反应总发生率为9.80%,对照组为16.00%,差异无统计学意义(P>0.05)。结论针灸辅助西药治疗消化性溃疡疗效显著,可降低炎症因子水平并促进胃肠蠕动,有利于其免疫功能的恢复。

Objective To observe the therapeutic effect of of acupuncture and moxibustion in the treatment of peptic ulcer and the changes of serum tumor necrosis factorα(TNF-α),interleukin(IL)-8 and gastrin(GAS)levels.Methods The 101 patients with peptic ulcer who were treated in the hospital from June 2018 to June 2020 were selected as the research objects,and were divided into observation group(n=51)and control group(n=50)according to the random number table method.Both groups were given basic treatment,the control group was treated with conventional western medicine,and the observation group was treated with acupuncture and moxibustion on the basis of the control group.The short-term(within 4 weeks)clinical effects of the two groups were compared,and the changes of inflammatory factors[TNF-α,IL-8,heat shock protein 60(HSP60)],serum gastrointestinal function indexes[GAS,somatostatin(SS),motilin(MTL)],peripheral blood T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+))and adverse reactions before and after treatment were recorded.Results After 4 weeks of treatment,the total effective rate of the observation group(94.12%)was significantly higher than that of the control group(80.00%),and the difference was statistically significant(P<0.05).After 4 weeks of treatment,the levels of IL-8,TNF-α,HSP60,GAS,MTL and CD8^(+)in the two groups were significantly lower than before treatment,while the levels of SS,CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)were all higher than before treatment,the range of changes in the observation group was greater,and the differences were statistically significant(P<0.05).During the course of treatment,the total incidence of adverse reactions in the observation group was 9.80%and that in the control group was 16.00%,the difference was not statistically significant(P>0.05).Conclusion Acupuncture and moxibustion assisted western medicine to treat peptic ulcers have a significant effect,can reduce inflammatory factors and promote gastrointestinal peristalsis,and is beneficial to the recovery of its immune function.
