宫颈癌患者放疗后放射性膀胱炎发生状况调查及相关因素分析

Investigation of Radiation Cystitis Status after Radiotherapy of Cervical Cancer and Analysis of Related Factors

目的分析宫颈癌患者放疗后放射性膀胱炎的发生状况及其相关影响因素。方法回顾性分析2006年6月至2019年5月在首都医科大学附属北京妇产医院妇瘤科接受治疗的204例宫颈癌患者的临床资料,分析宫颈癌治疗后放射性膀胱炎的发生状况及其与患者年龄、疾病分期、治疗方式和治疗后时间的相关性。结果宫颈癌患者放疗后放射性膀胱炎的总发生率为32.4%(66/204),其中Ⅰ~Ⅱ度放射性膀胱炎发生率为31.4%(64/204),Ⅲ度放射性膀胱炎发生率为1.0%(2/204)。多因素Logistic回归分析结果显示,疾病分期是宫颈癌放疗后放射性膀胱炎发生的相关因素(P<0.05),而年龄、治疗方式、治疗后时间不是宫颈癌放疗后放射性膀胱炎发生的相关因素(P>0.05)。结论宫颈癌放疗后(根治性放疗及术后辅助性放疗)放射性膀胱炎发生率较高,其中以Ⅰ~Ⅱ度放射性膀胱炎更常见。疾病分期是影响放射性膀胱炎发生的主要因素,而年龄、手术、同步化疗及治疗后时间均不是放射性膀胱炎发生的影响因素。

Objective To analyze the incidence and related factors of radiation cystitis in cervical cancer after radiotherapy.Methods The clinical data of 204 cases of cervical cancer treated in the Department of Gynecology and Oncology,Beijing Obstetrics and Gynecology Hospital Affiliated to Capital Medical University from Jun.2006 to May 2019 were retrospectively analyzed,to investigate the incidence of radiation cystitis after treatment of cervical cancer and its correlation with age,disease stage,treatment method and time after treatment.Results The total incidence of radiation cystitis in the cervical cancer patients after radiotherapy was 32.4%(66/204),of which the incidence of grade Ⅰ-Ⅱ radiation cystitis was 31.4%(64/204),and the incidence of grade Ⅲ radiation cystitis was 1.0%(2/204).Multivariate Logistic regression analysis showed that disease stage was the related factor of radiation cystitis after radiotherapy of cervical cancer(P<0.05),while age,treatment method and time after treatment were not the related factors(P>0.05).Conclusion The incidence of radiation cystitis after radiotherapy for cervical cancer(radical radiotherapy and postoperative adjuvant radiotherapy)is high,and the degreeⅠ-Ⅱradiation cystitis is more common.Disease stage is the main factor affecting the incidence of radiation cystitis,while age,surgery,concurrent chemotherapy and time after treatment are not the influencing factors.
