热性惊厥患儿脑缺氧损伤与血清BDNF、HSP70水平变化的研究

Study on hypoxia-induced cerebral injury and changes in serum BDNF and HSP70 levels in children with febrile convulsion

目的探讨热性惊厥患儿脑缺氧损伤与血清脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)、热休克蛋白70(heat shock protein 70,HSP70)水平变化情况。方法选取热性惊厥患儿70例作为研究组,根据患儿热性惊厥发作时间分为单纯惊厥组(45例)、惊厥持续状态组(25例);同时选取同期在健康体检儿童70例作为对照组。比较三组BDNF、HSP70水平、pH值、氧分压(arterial partial pressure of oxygen,PaO2)、二氧化碳分压(arterial partial pressure of carbon dioxide,PaCO2)、二氧化碳总量(total carbon dioxide,TCO2)、血气水平。结果惊厥持续状态组BDNF、HSP70水平高于对照组、单纯惊厥组(P<0.05);对照组与单纯惊厥组对比,两组pH值、PaO2、PaCO2、TCO2比较差异无统计学意义(P>0.05),惊厥持续状态组pH值、PaO2低于对照组和单纯惊厥组(P<0.05),惊厥持续状态组PaCO2、TCO2高于对照组和单纯惊厥组(P<0.05)。结论热性惊厥患儿脑缺氧损伤研究中发现发生惊厥持续状态的患儿脑缺氧损伤与BDNF、HSP70水平存在很大的相关性,应深入研究。

Objective To investigate hypoxia-induced cerebral injury and the changes of brain-derived neurotrophic factor(BDNF)and heat shock protein 70(HSP70)in children with febrile convulsion.Methods A total of 70 children with febrile convulsion were selected as the research group,and they were divided into simple convulsion group(n=45)and status convulsion(SC)group(n=25)according to the duration of febrile convulsion.In the meantime,70 healthy children undergoing physical examination in our hospital during the same period were selected as the control group.The serum levels of BDNF,HSP70,pH value,arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2),total carbon dioxide(TCO2),and blood gas levels were compared among three groups.Results The levels of BDNF and HSP70 in SC group were higher than those in control group and simple convulsion group(P<0.05);there was no significant difference between the control group and simple convulsion group with respect to pH value,PaO2,PaCO2 and TCO2(P>0.05).pH value and PaO2 of SC group were lower than those of control group(P<0.05),while PaCO2 and TCO2 of SC group were higher than those of control group and simple convulsion group(P<0.05).pH value and PaO2 of SC group were lower than those of simple convulsion group(P<0.05).Conclusion In the study of hypoxia-induced cerebral injury in children with febrile convulsion,it is found that there is a significant correlation of hypoxia-induced cerebral injury with BDNF and HSP70 levels in children with SC,which should be further studied.
