自制克氏针定位调节器在脊柱内镜手术中的应用

摘要 目的分析改良关节突截骨技术在椎间孔镜下治疗腰椎管狭窄症(PELD)的临床疗效及手术技巧。方法选取我院自2018年2月至2020年9月间根管狭窄型腰椎管狭窄症患者46例,其中男26例,女20例,年龄42~73岁,平均(56±7.1)岁。术前定位将1枚2.0 mm克氏针作为导针滑入椎间孔内,应用克氏针调节器固定针根据临床需要向背侧调节后,应用6.3 mm环锯做关节突截骨并进行手术。结果所有患者均顺利完成手术,手术时间平均(50.32±10.54)min,术中透视次数平均为(5.00±2.14)次。所有患者平均随访时间8.23个月(6~15个月)。术后各时间点VAS评分及ODI评分较术前均有显著改善,差异有统计学意义(P<0.001)。结论PELD术中应用克氏针调节器辅助一次关节突成形技术,在缩短手术定位时间的同时,减少了放射暴露,取得了良好的治疗效果,适合临床推广应用。
