女性青少年特发性脊柱侧凸患者骨质强度与脊柱侧凸角度的相关性分析

Correlation analysis between bone strength and scoliosis angle in female adolescent idiopathic scoliosis

目的探讨女性青少年特发性脊柱侧凸(adolescent idiopathic scoliosis,AIS)患者的骨质强度与脊柱侧凸角度的相关性。方法选取2018年3月—2019年10月在上海交通大学医学院附属新华医院康复医学科门诊初诊的女性AIS患者,收集年龄、身高、体质量、初潮年龄等一般资料;经全脊柱X线正位片评估脊柱侧凸严重程度(Cobb角)和骨骼成熟度(Risser征);超声骨强度仪检测骨质强度(超声声速值)。分析超声声速值与Cobb角的相关性。结果共纳入403例女性AIS患者,平均年龄(13.19±1.75)岁,Cobb角10°~59°。单因素线性相关分析结果显示超声声速值与Cobb角无相关性(r=-0.092,P=0.065);调整年龄、身高、体质量、Risser征等因素后,多元线性回归分析结果表明超声声速值与Cobb角呈显著负相关(β=-1.705,P=0.010)。结论脊柱侧凸角度是女性AIS患者骨质强度的影响因素,角度大则骨质强度较低。

Objective To investigate the correlation between bone strength and the scoliosis angle in female adolescent idiopathic scoliosis(AIS).Methods Newly diagnosed AIS female patients aged 10-17 in the Rehabilitation Department,Xinhua Hospital,Shanghai Jiao Tong University School of Medicine were selected from March 2018 to October 2019.General information such as age,height,weight,and age at menarche were collected.The severity of scoliosis(Cobb angle)and bone maturity(Risser sign)were evaluated by full spine X-ray;and bone strength speed of sound(SOS)was detected by ultrasound bone strength meter.And linear regression was used to analyze the correlation between SOS value and Cobb angle.Results A total of 403 female patients,aged(13.19±1.75)years old,with a Cobb angle of 10°-59°were included in this study.The crude analysis showed there was no correlation between the SOS value and the Cobb angle(r=-0.092,P=0.065).After adjustment for age,height,weight and Risser sign,the SOS value was negatively correlated with the Cobb angle in female AIS patients(β=-1.705,P=0.010).Conclusion The Cobb angle of female AIS is correlated with bone strength,and the bone strength is lower in patients with large Cobb angle.
