骨科手术机器人辅助下“8”字钢板螺钉置入治疗儿童下肢畸形 被引量:1

摘要 目的探讨骨科手术机器人辅助下“8”字钢板骨骺阻滞术治疗儿童下肢畸形的临床效果。方法回顾性分析2019年12月甘肃省中医院收治的2例下肢畸形患儿的临床资料,患儿均采用“天玑”骨科手术机器人辅助下“8”字钢板骨骺阻滞术。记录术中出血量、操作时间、X线透视次数、手术精度、导针置入调整次数以及不良事件的发生情况,随访期间观察患儿下肢步态及畸形矫正情况。结果2例手术均顺利完成,手术时间、术中出血量分别为45和40 min、30和25 mL,术中图像采集、定位、拍片均一次性完成。导针均成功置入,无需调整,置入螺钉与骺线平行,钢板位置满意。术后均随访12个月,随访期间未见手术相关并发症,末次随访时患儿下肢畸形改善明显,矫形效果满意。结论骨科手术机器人辅助下“8”字钢板骨骺阻滞术治疗儿童下肢畸形方法可行,置钉精确度高,安全性好。 Objective To investigate the clinical effect of eight-plate epiphyseal block assisted by orthopedic robot in the treatment of lower limb deformities in children.Methods Clinical data of 2 children with lower limb deformities treated in Gansu Provincial Hospital of TCM in December 2019 were retrospectively analyzed.Both of the children were treated with eight-plate epiphyseal block assisted by"Tianji"orthopedic robot.Estimate intraoperative blood loss,operation time,radiation time of X-ray fluoroscopy,the accuracy of operation,times of adjustment of guide needle placement and the occurrence of adverse events were recorded.The gait and deformity correction of lower extremities were observed during the follow-up.Results The operations were completed successfully,with the operation time of 45,40 min and estimate intraoperative blood loss of 30,25 mL respectively.The intraoperative image acquisition,location and photography were all completed at one time.The insertion procedure of the guide needle was accomplished once without any adjustment,the insertion screw was parallel to the epiphyseal line,and the position of plate fixation was satisfactory.No surgery related complications were found during the follow-up.Both children were followed up for 12 months.At the latest follow-up,the deformities of the lower extremities were significantly improved and the orthopedic effects were satisfactory for the two patients.Conclusion Orthopedic robot assisted eight-plate epiphyseal block in the treatment of lower limb deformities in children is feasible with high accuracy and good safety of screw placement.
