外周血中性粒细胞/淋巴细胞比值在腮腺恶性肿瘤诊断中的价值

Peripheral blood neutrophil-lymphocyte ratio in the diagnosis of parotid cancer

目的初步探讨术前外周血中性粒细胞/淋巴细胞比值(neutrophil-to-lymphocyte ratio,NLR)在腮腺恶性肿瘤患者诊断中的价值。方法回顾性分析2011年1月2020年8月中国人民解放军联勤保障部队第九〇〇医院耳鼻咽喉头颈外科收治且术后病理证实的41例腮腺恶性肿瘤患者和66例腮腺良性肿瘤患者的临床资料,记录并计算术前外周血NLR值,比较腮腺恶性肿瘤组和腮腺良性肿瘤组之间NLR的差异,通过ROC曲线获得腮腺恶性肿瘤和腮腺良性肿瘤患者之间术前外周血NLR的最佳临界值。结果腮腺恶性肿瘤组和良性肿瘤组NLR值分别为2.82±1.40、1.77±0.56,差异有统计学意义(t=5.459,P<0.01),NLR的曲线下面积为0.722,诊断腮腺恶性肿瘤的最佳临界值为2.441,敏感性为56.1%,特异性为90.9%。结论术前外周血NLR水平检测可作为评价腮腺肿物良恶性的指标之一,NLR值越高,肿瘤分期越高、淋巴结转移可能性越大。

OBJECTIVE To investigate the clinical significance of peripheral blood neutrophil-to-lymphocyte ratio(NLR)in the diagnosis of parotid cancer.METHODSThe clinical data of 41 parotid cancer and 66 parotid benign tumor cases newly diagnosed and confirmed by postoperative pathology at the Department of Otolaryngology Head and Neck Surgery of the 900th Hospital of Joint Logistic Support Force from January 2011 to August 2020 were analyzed retrospectively.The NLR were compared among different groups,and according to the area under the receiver-operating characteristic(ROC)curve,the optimal cut-off value was determined.RESULTS The NLR was 2.82±1.40 and 1.77±0.56 in the parotid cancer group and parotid benign tumor group respectively,the NLR had significant difference between the two groups(P<0.01).The area under curve(AUC)of NLR was 0.722,the optimal cut-off value was 2.441,the sensitivity was 56.1%,the specificity was 90.9%.CONCLUSIONNLR may be helpful to differentiate parotid cancer from parotid benign tumor.The higher NLR means the higher the clinical stage and the more possibility of lymphatic metastasis.
