摘要
目的探讨声门下吸引对重症患儿呼吸机相关性肺炎(VAP)的预防效果。方法选取上海交通大学附属儿童医院自2019年10月至2020年4月收治的57例重症患儿为研究对象。采用随机数字表法将其分为A组(n=31)与B组(n=26)。A组患儿行常规呼吸机辅助机械通气,并给予常规气管导管治疗。B组患儿行常规呼吸机辅助机械通气,并给予可冲洗的气管插管进行声门下吸引。记录并比较两组患儿VAP发生率、机械通气时间、重症监护病房(ICU)住院时间及住院费用。结果A组患儿VAP发生率、机械通气时间、ICU住院时间均大于B组,差异均有统计学意义(P<0.05)。两组患儿住院费用比较,差异无统计学意义(P>0.05)。结论声门下吸引可以预防重症患儿VAP的发生,缩短机械通气时间及ICU住院时间。
Objective To investigate the preventive effect of subglottic suction on ventilator-associated pneumonia(VAP)in severely ill children.Methods A total of 57 cases of critically ill children admitted to the Children's Hospital Affiliated to Shanghai Jiaotong University from October 2019 to April 2020 were selected as the research subjects.They were divided into group A(n=31)and group B(n=26)by random number table method.Children in group A were treated with conventional ventilator assisted mechanical ventilation and conventional tracheal catheter.Group B received conventional ventilator assisted mechanical ventilation and washeable tracheal intubation for subglottic suction.The incidence of VAP,duration of mechanical ventilation,length of stay in intensive care unit(ICU)and hospitalization costs were recorded and compared between the two groups.Results The incidence of VAP,duration of mechanical ventilation and length of ICU stay in group A were all higher than those in group B,with statistical significance(P<0.05).There was no significant difference in the cost of ICU hospitalization between the two groups(P>0.05).Conclusion Subglottic suction can prevent ventilator-associated pneumonia in severely ill children,reduce the incidence of VAP,shorten the duration of mechanical ventilation and ICU hospitalization.
