5,10-亚甲基四氢叶酸还原酶基因多态性和子痫前期的相关性 被引量:2

Correlation between MTHFR gene polymorphism and preeclampsia

摘要 目的分析5,10-亚甲基四氢叶酸还原酶(MTHFR)基因多态性和子痫前期的相关性。方法选取2018年9月-2019年9月在湖州市妇幼保健院分娩的300例孕妇,分为研究组(合并子痫前期的150例单胎妊娠孕妇)和对照组(不合并子痫前期的150例单胎妊娠孕妇)。比较两组孕妇叶酸水平、同型半胱氨酸(Hcy)水平,不同MTHFR基因型孕妇叶酸水平、Hcy水平。分析MTHER基因多态性和子痫前期的相关性。结果 MTHFR A1298C位点AC+CC基因型孕妇叶酸水平为(11.48±6.38)ng/ml,AA基因型孕妇叶酸水平为(11.01±3.87)ng/ml,差异有统计学意义(t=0.370,P=0.121)。MTHFR C677T位点CT+TT基因型孕妇Hcy水平为(8.93±2.67)μmol/L,CC基因型孕妇Hcy水平为(5.75±1.46)μmol/L,差异有统计学意义(t=6.510,P=0.009)。MTHFR A1298C位点AC+CC基因型孕妇Hcy水平为(8.89±3.54)μmol/L,AA基因型孕妇Hcy水平为(6.92±1.65)μmol/L,差异有统计学意义(t=3.580,P=0.008)。研究组MTHFR C677T CT突变孕妇患子痫前期的风险是对照组孕妇的4.240倍,MTHFR C677T TT突变孕妇患子痫前期的风险是对照组孕妇的6.102倍。MTHFR A1298C CC突变是子痫前期发生的高风险因子之一,研究组A1298C CC突变孕妇患子痫前期的风险是对照组孕妇的3.643倍。结论子痫前期孕妇死亡率较高,MTHFR基因C677T和A1298C位点与子痫前期的发生相关,早期检测孕妇MTHFR基因有助于防止子痫前期的发生和发展。 Objective To analyze the correlation between 5,10-methylenetetrahydrofolate reductase(MTHFR) gene polymorphism and preeclampsia.Methods A total of 300 pregnant women were selected from Huzhou Municipal Maternal and Child Health Care Hospital from September 2018 to September 2019,then they were divided into study group(150 single pregnant women with preeclampsia) and control group(150 single pregnant women without preeclampsia). The levels of folic acid and homocysteine(Hcy) in the two groups were compared,the levels of folic acid and Hcy in pregnant women with different MTHFR genotypes were compared. The correlation between MTHFR gene polymorphism and preeclampsia was analyzed.Results The level of folic acid in pregnant women with MTHFR A1298 C locus AC+CC genotypes was(11.48±6.38)ng/ml,the level of folic acid in pregnant women with MTHFR A1298 C locus AA genotype was(11.01±3.87)ng/ml,there was statistically significant difference(t=0.370,P=0.121). The risk of preeclampsia in pregnant women with MTHFR C677 T CT mutation in study group was 3.240 times more than that in control group,the risk of preeclampsia in pregnant women with MTHFR C677 T TT mutation in study group was 5.102 times more than that in control group. MTHFR A1298 C CC mutation was one of the high-risk factors of preeclampsia,the risk of preeclampsia in pregnant women with A1298 C CC mutation in study group was 2.643 times more than that in control group.Conclusion The mortality rate of pregnant women with preeclampsia is high,MTHFR C677 T and A1298 C loci are closely correlated with preeclampsia,early detection of MTHFR gene in pregnant women can prevent occurrence and development of preeclampsia.
