克罗恩病患者循环LncRNA-MEG3的表达水平及意义

Expression level and significance of LncRNA-MEG3 in blood circulation of patients with Crohn’s disease

目的探究克罗恩病(CD)患者循环长链非编码RNA(LncRNA)-母系表达基因3(MEG3)的表达水平及意义。方法选取2017年10月-2019年12月郑州大学附属郑州中心医院就诊的CD患者78例作为研究对象(观察组),根据CD活动指数(CDAI)分为缓解组(缓解期,CDAI<150分)40例和活动组(活动期,CDAI≥150分)38例。另选取同期健康体检者80人作为对照组。采用Pearson法分析CD患者血清LncRNA-MEG3表达水平与CDAI、CD简化内镜评分(SES-CD)的相关性,采用受试者工作特征(ROC)曲线评估血清LncRNA-MEG3水平对CD及CD疾病活动性的诊断价值。结果观察组血清LncRNA-MEG3表达水平较对照组显著降低(P<0.05);活动组血清LncRNA-MEG3表达水平较缓解组显著降低(P<0.05)。CD患者血清LncRNA-MEG3表达水平与CDAI、SES-CD均呈显著负相关(r=-0.514、-0.476,P均<0.05)。血清LncRNA-MEG3表达水平诊断CD的ROC曲线下面积为0.858(95%CI:0.7980.918),截断值为1.657,敏感度为84.6%,特异性为80.0%,约登指数为0.646;血清LncRNA-MEG3表达水平诊断CD疾病活动性的ROC曲线下面积为0.850(95%CI:0.7630.937),截断值为1.400,敏感度为75.0%,特异性为84.2%,约登指数为0.592。结论CD患者血清LncRNA-MEG3表达水平显著降低,与疾病活动性密切相关,对CD具有很好的诊断价值,可能是区分CD患者活动性的潜在生物学标志物。

Objective To investigate the expression level and significance of long non-coding RNA(LncRNA)-maternal expression gene 3(MEG3)in the blood circulation of patients with Crohn’s disease(CD).Methods 78 CD patients in Zhengzhou Central Hospital Affiliated to Zhengzhou University from October 2017 to December 2019 were selected as study objects(observation group),and according to CD activity index(CDAI),they were divided into remission group(remission period,CDAI<150 points)and active group(activity period,CDAI≥150 points).80 healthy persons in the same period were selected as the control group.Pearson’s method was used to analyze the correlations between the expression level of serum LncRNA-MEG3 and CDAI,simplified endoscopy score of CD(SES-CD);the receiver operating characteristic(ROC)curve was used to evaluate the diagnostic values of serum LncRNA-MEG3 level for CD and CD disease activity.Results The expression level of serum LncRNA-MEG3 in the observation group was significantly lower than that in the control group(P<0.05);the expression level of serum LncRNA-MEG3 in active group was significantly lower than that in remission group(P<0.05);the expression level of serum LncRNA-MEG3 was negatively correlated with CDAI and SES-CD(r=-0.514,-0.476,P<0.05);the area under ROC curve of CD diagnosed by expression level of serum LncRNA-MEG3 was 0.858(95%CI:0.798-0.918),with the cutoff value of 1.657,the sensitivity of 84.6%,the specificity of 80.0%,and the Youden index of 0.646.The area under ROC curve of CD activity diagnosed by expression level of serum LncRNA-MEG3 was 0.850(95%CI:0.763-0.937),with the cutoff value of 1.400,the sensitivity of 75.0%,the specificity of84.2%,and the Youden index of 0.592.Conclusion The expression level of serum LncRNA-MEG3 in CD patients was significantly decreased,which was closely related to disease activity;it had a good diagnostic value for CD,and might be a potential biomarker to distinguish CD patients’activity.
