九针围刺针法联合火针腰夹脊穴治疗气滞血瘀型腰椎间盘突出症临床研究

Clinical Study of Nine Needles Encircling Needling Combined with Fire Needling Lumbar Jiaji Points in the Treatment of LDH of Qi Stagnation and Blood Stasis

目的:探讨九针围刺针法联合火针腰夹脊穴治疗气滞血瘀型腰椎间盘突出症的疗效。方法:选取本院收治的气滞血瘀型腰椎间盘突出症患者120例,随机分为两组各60例。对照组给予火针腰夹脊穴治疗,治疗组在对照组基础上给予九针围刺针法,比较治疗后的临床疗效、中医症候评分、VAS评分、JOA评分、ODI评分及血清炎性因子水平。结果:经过治疗后,治疗组总有效率比对照组总有效率显著较高(P<0.05);两组肢体发凉、腰脊疼痛、神疲和腰膝酸软等症候评分较治疗前显著升高(P<0.05),且治疗组升高程度较大(P<0.05);两组VAS评分和ODI评分较治疗前明显降低、JOA评分显著升高(P<0.05),且治疗组改善程度较大(P<0.05);两组血清IL-6、IL-10和TNF-α水平较治疗前明显降低(P<0.05),且治疗组降低程度较大(P<0.05)。结论:采用九针围刺针法联合火针腰夹脊穴治疗气滞血瘀型腰椎间盘突出症具有较好的临床疗效,能够改善患者腰椎功能,值得在临床上推广应用。

Objective:To investigate the clinical efficacy of nine needles encircling needling combined with fire needling lumbar Jiaji points in the treatment of lumbar disc herniation(LDH)of qi stagnation and blood stasis and its influences to JOA score.Methods:120 cases of LDH of qi stagnation and blood stasis were randomly divided into the control group and the treatment group,with 60 cases in each group.The control group was treated with fire needling lumbar Jiaji points;on which basis,the treatment group was also treated with nine needles encircling needling method.The clinical efficacy,the scores of TCM symptoms,VAS,JOA and ODI,as well as the levels of serum inflammatory factors were compared between the two groups after the treatment.Results:The total effective rate was significantly higher in the treatment group than that in the control group(P<0.05).The scores of body cool feeling,lumbar spinal pain,fatigue,lumbar and knee tenderness and other symptoms were significantly increased in the two groups after the treatment(P<0.05);of which the increases in the treatment group were more significant(P<0.05).The scores of VAS and ODI were significantly reduced,and JOA score was significantly increased in the two groups after the treatment(P<0.05);of which the improvements in the treatment group were more significant(P<0.05).The serum levels of IL-6,IL-10 and TNF-a were significantly decreased in both groups after the treatment(P<0.05);of which the decreases in the treatment group were more significant(P<0.05).Conclusion:Nine needles encircling needling combined with fire needling lumbar Jiaji points has a significant clinical effect on LDH of qi stagnation and blood stasis,which can improve the lumbar function of the patients.The therapy is worthy of clinical application.
