老年单纯性肾囊肿人群并发高血压及肾功能下降的危险因素分析

Analysis of risk factors for hypertension and decreased renal function in elderly patients with simple renal cysts

目的探讨老年人单纯性肾囊肿(SRC)与高血压和肾功能下降的关系。方法2012年1月-12月,纳入上海市闵行区江川路街道≥60岁体检人群14173例。根据肾脏B超结果分为SRC组和无SRC组,比较2组的年龄、性别、体质量指数、既往史(高血压、糖尿病、高脂血症)、血常规、血尿素氮(BUN)、血肌酐(SCr)、血尿酸(SUA)、空腹血糖、胆固醇(TC)1三酰甘油(TG)、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇及尿常规等。分析SRC合并高血压和肾功能下降的影响因素。结果最终纳入12646例受试者,年龄60~97岁,平均(69.3±7.2)岁,SRC患病率为13.68%(1730/12646)。年龄、性别、SCr、TC、肾结石和高血压是SRC的独立危险因素。Logistic回归分析显示,SUA、TG、糖尿病及多发肾囊肿是SRC合并高血压的独立危险因素,而年龄、SCr、BUN、SUA、蛋白尿和高血压是SRC合并肾功能下降的独立危险因素。结论SUA、TG、糖尿病及多发肾囊肿可能是SRC合并高血压的独立危险因素,年龄、SCr、BUN、SUA、蛋白尿和高血压可能是SRC合并肾功能下降的独立危险因素。

Objective To explore the relationship between simple renal cysts(SRC)and hypertension and decreased renal function in the elderly.Methods 14173 physical examination people aged≥60 years old in Jiangchuan street of Minhang district of Shanghai city from January to December 2012 were enrolled.According to the results of renal B-ultrasound,they were divided into SRC group and non-SRC group.The age,gender,body mass index,past history(hypertension,diabetes,hyperlipidemia),blood routine,blood urea nitrogen(BUN),serum creatinine(SCr),blood uric acid(SUA),fasting blood glucose,cholesterol(TC),three glycerol(TG),high-density lipoprotein cholesterol,low density lipoprotein cholesterol,and urine routine were compared between the two groups.The influencing factors of SRC complicated with hypertension and decreased renal function were analyzed.Results Finally,12646 subjects were enrolled,aged 60~97 years,with an average of(69.3±7.2)years.The prevalence of SRC was 13.68%(1730/12646).Age,gender,SCr,TC,kidney stones and hypertension were independent risk factors for SRC.Logistic regression analysis showed that SUA,TG,diabetes and multiple renal cysts were independent risk factors for SRC with hypertension,while age,SCr,BUN,SUA,proteinuria and hypertension were independent risk factors for SRC with decreased renal function.Conclusion SUA,TG,diabetes and multiple renal cysts may be independent risk factors for SRC combined with hypertension,and age,SCr,BUN,SUA,proteinuria and hypertension may be independent risk factors for SRC combined with decreased renal function.
