围术期前置审方系统拦截医嘱非甾体抗炎药不合理使用点评

Investigation and Analysis of Unreasonable Use of Non-steroidal Anti-inflammatory Drugs in Perioperative Period

目的:通过对本院围术期非甾体抗炎药(NSAIDs)不合理使用作调查分析,为促进其围术期合理使用提供参考。方法:采用回顾性调查方法,选取2019年1~12月前置审方打回的围术期NSAIDs药医嘱,对其手术类型的分布、使用中存在的问题以及干预情况进行统计分析。结果:在打回的429条医嘱中,以骨科手术不合理用药最多,占22.84%;不合理使用类型,以2种NSAIDs联用最多,占51.05%;经干预,不合理医嘱数量逐渐减少,修改率逐渐升高。结论:NSAIDs药在围术期使用中存在一定问题,建议制定该类药围术期使用规范和事前审方干预,促进其合理使用。

Objective:The investigation and analysis of the unreasonable use of non-steroidal anti-inflammatory drugs(NSAIDs)in a hospital was done in order to provide reference for promoting the rational use of NSAIDs in the perioperative period.Methods:By retrospective investigation,the doctor’s orders of perioperative NSAIDs from January to December 2019 were selected,and the distribution of operation types,the problems in use and the interventions of pre-dispensing were statistically analyzed.Results:Among the 429 orders rejected by pharmacists,the most frequent unreasonable use of NSAIDs appeared in orthopedic surgeries,accounting for 22.84%;in terms of unreasonable use types,the combination of two kinds of NSAIDs accounted for 51.05%.As the result of intervention,the number of unreasonable orders gradually decreased,and the revision rate gradually increased.Conclusion:There are some problems in the perioperative use of NSAIDs.It is suggested that the perioperative use of NSAIDs should be standardized and the training of doctors and the intervention of pre-dispensing should be strengthened to promote the rational use of NSAIDs during the perioperative period.
