双环法、欧米伽法与Mammotome法在不同分度男性乳房发育症的治疗效果评价

Evaluation of the Therapeutic Effect of Double Loop Method,Omega Method and Mammotome Method in Different Grades of Gynecomastia

目的观察双环法、欧米伽法与Mammotome法在不同分度男性乳房发育症的治疗效果并进行评价。方法选择2015年5月至2020年12月广州市第一人民医院收治的70例男性乳房发育患者,其中36例Ⅲ度男性乳腺增生采取双环法进行手术、28例Ⅱ度采取欧米伽法和6例Ⅰ度采取Mammotome法手术治疗。观察对比3组患者术后疼痛评分、出现皮下血肿、皮下积液、伤口感染等并发症等比较三种术式的治疗效果。根据3组患者术后3个月门诊随访期间乳房美学指标比较患者对治疗的效果评价。结果3组患者疼痛评分和并发症的发生无统计学差异,Mammotome法治疗男性乳房发育症较双环法和欧米伽法瘢痕小,基本达到几乎看不到,差异具有统计学意义(P=0.0001)。结论不同程度的男性乳房发育患者采取双环法组、欧米伽法组和Mammotome法组均能达到治疗效果,切实可行,对于术后瘢痕情况,Mammotome法组瘢痕小且更隐蔽,值得临床借鉴。

Objective To observe and evaluate the therapeutic effects of double-loop method,omega method and Mammotome method in different grades of gynecomastia.Methods A total of 70 patients with gynecomastia admitted to Guangzhou First People's Hospital from May 2015 to December 2020 were selected.Among them,36 patients with third-degree gynecomastia were operated on with double-circle method,and 28 patients with second-degree received omega method and six cases of first degree were treated with Mammotome method.Observation and comparison of the three groups of patients with postoperative pain scores,subcutaneous hematoma,subcutaneous fluid,wound infection and other complications to compare the therapeutic effects of the three surgical methods.According to the three groups of patients during the three-month outpatient follow-up period,the evaluation of the effect of the patients on the treatment was compared.Results There was no statistically significant difference in pain scores and complications between the three groups of patients.Mammotome's treatment of gynecomastia had smaller scars than the double-circle method and omega method,and the scars were almost invisible.The difference was statistically significant(P=0.0001).Conclusion The double-circle method group,omega method group and Mammotome method group can achieve the therapeutic effect for patients with different degrees of gynecomastia,and it is feasible.For postoperative scars,the Mammotome method group has smaller scars and more concealed,which is worthy of clinical reference.
