NF-κB/uPA在胎盘植入性疾病患者血清中的表达及意义

摘要 目的探讨核转录因子(NF-κB)、尿激酶型纤溶酶原激活物(uPA)在胎盘植入性疾病患者外周血、脐血中的表达情况及其意义。方法选取住院分娩的胎盘植入性疾病患者30例作为研究对象,15例胎盘粘连患者为胎盘粘连组,15例胎盘植入患者为胎盘植入组,同时选取15例正常待产无并发症并要求行剖宫产手术的患者为对照组。采用ELISA法检测3组患者外周血及脐血中NF-κB、uPA水平的表达。结果3组患者外周血及脐血中NF-κB表达比较,胎盘植入组外周血中NF-κB表达水平明显高于胎盘粘连组及对照组,差异有统计学意义(F=5.5,P<0.05);胎盘植入组脐血中NF-κB表达水平明显高于胎盘粘连组及对照组,差异有统计学意义(F=5.3,P<0.05)。3组患者外周血及脐血中uPA表达比较,胎盘植入组外周血中uPA表达水平明显高于胎盘粘连组及对照组,差异有统计学意义(F=4.11,P<0.05);胎盘植入组脐血中uPA表达水平高于胎盘粘连组与对照组,差异有统计学意义(F=4.57,P<0.05)。结论胎盘植入性疾病患者外周血及脐血中NF-κB/uPA均呈高表达,二者在胎盘滋养细胞向子宫肌层的黏附、增殖及侵袭过程中发挥一定作用,而导致胎盘植入性疾病的发生。 Objective To explore the expression and significance of nuclear factor-κB NF-κB and urokinase plasminogen activator uPA in peripheral blood and cord blood of patients with placental implant disease.Methods Thirty patients with placental implant disease who hospitalized were selected as the research objects:15 patients with placental adhesions were in the placental adhesion group,and 15 patients with placenta accreta were in the placental implant group.A total of 15 patients who were expected to give birth without complications and required to undergo cesarean section as the control group.ELISA method was used to detect the expression of NF-κB and uPA in peripheral blood and cord blood of the three groups of patients.Results Comparison of the expression of NF-κB in the peripheral blood and cord blood of the three groups of patients,the expression level of NF-κB in the peripheral blood of the placental implantation group was significantly higher than that of the placental adhesion group and the control group(P<0.05);The expression level of NF-κB in the cord blood of the placental implantation group was significantly higher than that of the placental adhesion group and the control group(P<0.05);Comparison of uPA expression in peripheral blood and cord blood of the three groups of patients,the expression level of uPA in peripheral blood of the placenta accreta group was significantly higher than that of the placental adhesion group and the control group(P<0.05);The expression level of uPA in cord blood in the group was higher than that in the placental adhesion group and the control group(P<0.05).Conclusion NF-κB/uPA are highly expressed in peripheral blood and cord blood of patients with placental implant disease,and both play a certain role in the adhesion,proliferation and invasion of placental trophoblast cells to the myometrium,leading to placental implant disease happened.
