观察双层筋膜覆盖Medpor耳支架一期耳再造术的临床效果

Observe the Effect of One-Stage Ear Reconstruction with Medpor Stent Covered with Double Layer Fascia

目的:探究双层筋膜覆盖Medpor耳支架一期耳再造术的临床效果。方法:选择154例2017年1月~2018年9月在本院进行双层筋膜覆盖Medpor耳支架一期耳再造术治疗的先天性小耳畸形患者为研究对象,所有患者均实施双层筋膜覆盖Medpor耳支架一期耳再造术治疗,统计患者治疗及康复效果。结果:所有患者实施再造耳治疗后均成功成活,术后随访12个月,均未出现过敏或者排异等不良反应,其中150例患者再造耳廓质感柔软、形态逼且耳廓立体感较强,治疗后颅耳角与健康一侧耳廓相对称。剩余4例耳廓恢复不佳患者耳廓中有1例再造耳皮瓣存活效果不佳,可见明显支架外露情况;3例患者恢复不佳原因由于患者自身瘢痕体质所致手术切口出现瘢痕增生,手术后耳廓形态不佳。结论:双层筋膜覆盖Medpor耳支架一期耳再造术在先天性小耳畸形患者治疗中的应用能够通过再造耳方式实现治疗,且双层筋膜覆盖Medpor耳支架的柔软性较好,能够避免外耳轮支架外露,治疗效果及美观性均有保证。

Objective:Explore the effect of one-stage ear reconstruction with double-layer fascia covered Medpor stent.Methods:154 patients with congenital microtia treated by one-stage auricular reconstruction with Medpor auricular stent covered by double-layer fascia from January 2017 to September 2018 were selected as the study object,all patients underwent one-stage ear reconstruction with Medpor stent covered with double fascia,statistical treatment effect.Results:All patients successfully survived after the treatment of reconstructed ears.After 12 months of follow-up,there were no adverse reactions such as allergies or rejection.Among them,150 patients had soft and soft texture of the reconstructed auricles.The shape is compelling and the auricle has a strong sense of three-dimensionality.After the treatment,the cranial ear angle is symmetrical to the healthy side of the auricle.Among the remaining 4 patients with poor recovery of the auricle,1 of the reconstructed auricle flaps had poor survival,with obvious stent exposure;3 cases of poor recovery were due to scar hyperplasia in the surgical incision caused by the patient’s own scar constitution.Poor shape of the auricle after surgery.Conclusion:The application of double-layer fascia-covered Medpor ear stent in the treatment of congenital microtia can be achieved by reconstructing ears,and the double-layer fascia-covered Medpor ear stent has better flexibility and can be avoided.The external helix bracket is exposed,and the therapeutic effect and aesthetics are guaranteed.
