摘要
目的:探讨胎心监护仪联合B超仪器对判断胎儿窘迫的临床价值。方法:收集2018年3月~2020年3月在本院分娩的孕妇为观察对象,根据产妇产后脐动血pH分为观察组(pH<7.1,发生胎儿窘迫)和对照组(pH≥7.1,未发生胎儿窘迫)。对比胎儿窘迫组和对照组胎心监护仪监测结果;胎儿窘迫组和对照组B超结果;胎儿窘迫组和对照组不同产程脐血流S/D值。结果:通过对比胎心监护仪监测结果,发现胎儿窘迫组发生NST有反应型及可疑型的比例低于对照组,NST无反应型及胎心异常发生率高于对照组,两组比较有差异(P<0.05);胎儿窘迫组B超结果中S/D、PI及RI均高于对照组,两组比较有差异(P<0.05);胎儿窘迫组潜伏期、活跃期及第二产程的血流S/D值高于对照组,两组临产前血流S/D值结果比较无差异(P>0.05)。结论:胎心监护仪联合B超仪器对预测和判断胎儿窘迫的发生有积极的意义。
Objective:To explore the clinical value of fetal heart monitor combined with B-ultrasound device in the determination of fetal distress.Methods:The patients of pregnant women in our hospital from March 2018 to March 2020 were collected and divided into study group(pH<7.1,with fetal distress)and control group(pH≥7.1,without fetal distress)according to the pH value of postpartum umbilical blood.Compared fetal distress group and control group fetal heart monitor monitoring results;B ultrasound results of fetal distress group and control group;S/D values of umbilical cord blood flow during different stages in fetal distress group and control group.Results:By comparing the monitoring results of fetal heart monitor,it was found that the proportion of NST reactive type and suspicious type in fetal distress group was lower than that in control group,and the incidence of NST reactive type and abnormal fetal heart was higher than that in control group.There were differences between the two groups(P<0.05).In the fetal distress group,B-ultrasound results of S/D,PI and RI were higher than those of the control group,and there were differences between the two groups(P<0.05).The blood flow S/D values at the incubation period,active stage and the second stage of labor in the fetal distress group were higher than those in the control group,and there was no difference in the results of blood flow S/D values before labor between the two groups(P>0.05).Conclusion:The fetal heart monitor combined with B-ultrasound is of positive significance in predicting and judging the occurrence of fetal distress.
