陕西省介入放射学工作人员眼晶状体受照剂量水平 被引量:3

Radiation dose level of lens among interventional radiology workers in Shaanxi Province

摘要 目的了解陕西省介入人员眼晶状体受照剂量水平,对不同级别医疗机构介入放射学工作人员眼晶状体剂量进行监测,指导和改进介入放射学工作人员的管理和防护。方法 2019年8—10月,随机选择陕西省4家不同级别综合医院介入放射学工作人员作为监测对象,对其眼晶状体受照剂量进行测量和分析。结果陕西省不同级别医院、科室介入人员眼晶状体受照剂量差异较大,90天累计人均曝光时间最高者是介入科医师(18.0 h),人均眼晶状体受照剂量最高的是心内科医师(0.93 mSv);52.54%的介入人员左侧眼晶状体受照剂量高于右侧,左侧眼晶状体剂量与右侧比较,差异无统计学意义(P>0.05);被监测的59位介入人员眼晶状体集体剂量为62.92 mSv,平均当量剂量为0.32 m Sv,3位介入人员眼晶状体当量剂量>5 mSv,市级医院人均眼晶体当量剂量最高(0.57 mSv),其次是省级医院(0.12 mSv),县级医院最低(0.02 m Sv)。结论按照相同的工作量估计陕西省5.08%的介入人员在不佩戴铅眼镜的情况下眼晶体年受照剂量超过国际原子能机构规定的国际限值标准(20 mSv)。由于不同级别医院、不同科室的介入人员从事手术类型、术中职责、手术难易程度及累积曝光时间长短不同,介入人员眼晶状体的受照剂量差异较大。提示医疗机构应合理安排介入手术,提高术者的手术熟练程度,尽可能减少曝光时间。同时应重视眼部防护,加强防护管理,尽量减少职业人员眼晶状体的受照剂量。 Objective To understand the lens dose levels of interventional radiology workers in Shaanxi Province,monitor the lens dose of interventional radiology workers in different levels of medical institutions,and guide and improve the management and protection of interventional radiology workers. Methods Interventional radiology workers from four general hospitals at different levels in Shaanxi Province were randomly selected as monitoring subjects from August to October 2019,and the lens dose was measured and analyzed. Results Intraocular lens exposure dose of interventional radiology workers in different departments and hospitals at different levels in Shaanxi Province varied greatly. The highest cumulative exposure time per capita in 90 days was interventional physicians(18.0 h),and the highest lens exposure dose per capita was cardiologists(0.93 mSv). In 52.54% of the participants,the lens dose of the left eye lens was higher than that of the right eye,and there was no statistically significant difference in the lens dose between left eye and right eye(P>0.05). The collective dose of lens of 59 interventional subjects was 62.92 m Sv,the mean equivalent dose was 0.32 mSv,and the equivalent dose of lens in 3 interventional subjects was more than 5 mSv.Lens equivalent dose per capita was the highest in municipal hospitals(0.57 mSv),followed by provincial hospitals(0.12 m Sv),and the county hospitals was the lowest(0.02 mSv). Conclusion According to the same workload,it is estimated that the annual lens exposure dose of 5.08% of the interventional workers in Shaanxi Province exceeds the international limit(20 mSv) stipulated by the International Atomic Energy Agency(IAEA) without wearing lead glasses. The exposure dose of the lens of the interventional workers in different departments and hospitals at different levels is different due to the different types of operation,intraoperative re sponsibilities,difficulty of operation and cumulative exposure time. It is suggested that the medical institutions should arrange the interventional operation reasonably,improve the surgical proficiency and reduce the exposure time as much as possible,at the same time, pay attention to the protection of eyes,strengthen the management of protection,and try to reduce the exposure dose of eye lens of occupational group.
