腹部CT对结肠肿瘤性肠梗阻的诊断价值

The diagnostic value of abdominal CT in colonic neoplastic intestinal obstruction

目的评价腹部CT对结肠肿瘤性肠梗阻的诊断价值,为结肠肿瘤性肠梗阻诊治工作提供参考。方法80例结肠肿瘤性肠梗阻患者,均采用腹部CT、X线检测。比较两种检测方法梗阻诊断情况(梗阻诊断准确率、梗阻部位确定率、图像清晰度、舒适度)及梗阻部位检出情况。结果腹部CT梗阻诊断准确率100.00%、梗阻部位确定率96.25%、图像清晰率91.25%、舒适度85.00%均高于X线的85.00%、80.00%、65.00%、55.00%,差异均具有统计学意义(P<0.05)。腹部CT检测梗阻部位为升结肠、降结肠、乙状结肠、横结肠、结肠肝曲、结肠脾曲、结肠和直肠交界的检出率均略高于X线检测,但差异无统计学意义(P>0.05)。结论结肠肿瘤性肠梗阻诊断方面,腹部CT检查可明确检出病变、梗阻部位等情况,患者检查舒适度理想,助于临床工作的有效开展。

Objective To evaluate the diagnostic value of abdominal CT in colonic neoplastic intestinal obstruction,so as to provide reference for diagnosis and treatment of colonic neoplastic intestinal obstruction.Methods There were 80 patients with colonic neoplastic intestinal obstruction,and all were detected by abdominal CT and X-ray.The obstruction diagnosis(diagnostic accuracy of obstruction,determination rate of obstruction location,image clarity and comfort)and the detection of obstruction location were compared between the two detection methods.Results The diagnostic accuracy rate of obstruction,determination rate of obstruction location,image clarity rate and comfort level by abdominal CT were 100.00%,96.25%,91.25%and 85.00%,which were all higher than 85.00%,80.00%,65.00%and 55.00%by X-ray,and the difference was statistically significant(P<0.05).In terms of detection of obstruction sites of ascending colon,descending colon,sigmoid colon,transverse colon,colon liver flexure,colon splenic flexure,colon and rectum junction,the detection rate of abdominal CT was slightly higher than that of X-ray,but the difference was not statistically significant(P>0.05).Conclusion In the diagnosis of colonic neoplastic intestinal obstruction,abdominal CT can clearly detect the lesions,obstruction sites,and the patient is comfort during examination,which is conducive to the effective development of clinical work.
