中医外治法辅助治疗糖尿病视网膜病变的疗效观察

Observation on the efficacy of external treatment of traditional Chinese medicine as an adjunct treatment for diabetic retinopathy

目的研究分析中医外治法辅助治疗糖尿病视网膜病变(DR)的临床疗效。方法回顾性分析2019年9月至2020年9月在广东省中医院住院部收治的气阴两虚、瘀阻脉络证的DR患者80例,按治疗方法分为两组,各40例。对照组男22例,女18例,年龄(55.84±15.17)岁;治疗组男19例,女21例,年龄(57.55±16.22)岁。对照组予康柏西普注射液玻璃体腔注射治疗,并进行糖尿病教育、糖尿病饮食和血糖控制,治疗组在对照组基础上联合中医外治法。对治疗前后患者最佳矫正视力(BCVA)、中央黄斑厚度(CMT)进行评价。结果治疗前,两组DR患者BCVA比较,差异无统计学意义(P>0.05);两组DR患者BCVA治疗后均较本组治疗前提高,且观察组高于对照组,差异均有统计学意义(均P<0.05)。治疗前,两组DR患者CMT比较,差异无统计学意义(P>0.05);治疗后,观察组CMT为(263.60±64.09)μm,低于对照组(308.89±53.94)μm,差异有统计学意义(P<0.05)。观察组治疗总有效率为92.5%(37/40),高于对照组80.0%(32/40),差异有统计学意义(χ2=8.680,P<0.05)。结论中医外治法辅助治疗DR的临床疗效好,值得临床推广。

Objective To investigate the efficacy of external treatment of traditional Chinese medicine(TCM)for diabetic retinopathy.Methods A total of 80 patients diagnosed as diabetic retinopathy with stasis obstruction and deficiency of both Qi and Yin were included from September 2019 to September 2020 in the inpatient department of Guangdong Provincial Hospital of Traditional Chinese Medicine,and were divided into two groups according to treatment methods,40 cases in each group.In the control group,there were 22 males and 18 females,with an age of(55.84±15.17)years old.In the treatment group,there were 19 males and 21 females,with an age of(57.55±16.22)years old.The control group received intravitreal injection with conbercept injection,and diabetic education,diabetic diet,and blood sugar control were performed;the treatment group was given external TCM treatment on the basis.The best corrected visual acuity(BCVA)and central macular thickness(CMT)were assessed at pre and post-treatment.Results Before the treatment,there was no statistically significant difference in the BCVA between the two groups(P>0.05);after the treatment,the BCVA in the two groups were improved compared with those before the treatment,and that in the observation group was higher than that in the control group,with statistically significant differences(all P<0.05).Before the treatment,there was no statistically significant difference in the CMT between the two groups(P>0.05);after the treatment,the CMT of the observation group was(263.60±64.09)μm,lower than that of the control group[(308.89±53.94)μm],with a statistically significant difference(P<0.05).The total effective rate of the observation group was 92.5%(37/40),which was higher than that of the control group[80.0%(32/40)],with a statistically significant difference(χ2=8.680,P<0.05).Conclusion External TCM treatment has good clinical efficacy as an adjuvant therapy for diabetic retinopathy,worthy of clinical promotion.
