解毒化瘀方联合伊马替尼对急性淋巴细胞白血病患者心血管功能及免疫功能的影响

Effect of Jiedu Huayu Recipe Combined with Imatinib on CardiovasculaRFunction and Immune Function foRPatients with Acute Lymphoblastic Leukemia

目的:观察解毒化瘀方联合伊马替尼对急性淋巴细胞白血病(Acute Lymphoblastic Leukemia,ALL)患者心血管功能及免疫功能的影响。方法:选取2017年10月~2019年9月在我院治疗的ALL患者62例。随机将患者分为对照组(n=31)和观察组(n=31),对照组采用常规药物化疗方案,同时给予伊马替尼治疗,观察组在对照组的基础上还增加解毒化瘀方治疗,观察两组患者的临床疗效、中医症状评分、血常规检测结果,检测治疗前后两组心血管功能指标[每分心搏出量(CO)、血流加速时间(ACT)、最大血流速度(Vmax)、左室射血分数(LV EF)]及免疫功能相关指标[白介素-4(IL-4)、白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)及干扰素-γ(IFN-γ)]的水平。结果:观察组临床有效率为90.32%,明显高于对照组70.97%(P<0.05);治疗后,观察组中医症状评分、白细胞计数(WBC)与对照组相比显著降低(P<0.05),血红蛋白(Hb)、血小板计数(PLT)、红细胞数(RBC)均显著升高(P<0.05);治疗后,观察组ACT与对照组相比显著延长(P<0.05),CO、Vmax及LVEF水平显著降低;治疗后,观察组IL-8水平与对照组相比显著降低(P<0.05),IL-4、TNF-α及IFN-γ水平均显著升高(P<0.05)。结论:解毒化瘀方联合伊马替尼治疗ALL疗效显著,有效缓解临床症状,改善心血管功能,调节免疫功能。

Objective:To study the effect of Jiedu Huayu Recipe combined with Imatinib on cardiovasculaRfunction and immune function foRpatients with acute lymphoblastic leukemia(ALL).Methods:62 patients with ALL treated in ouRhospital from OctobeR2017 to SeptembeR2019 were selected.The patients were randomly divided into a controlled group(n=31)and an observation group(n=31).The controlled group was treated with routine drug chemotherapy and Imatinib at the same time,and the observation group was treated with Jiedu Huayu Recipe on the basis of the controlled group.The clinical efficacy,TCM symptom score and blood routine test results of the two groups were observed.The cardiovasculaRfunction indexes[(CO)peRminute stroke volume,blood flow acceleration time(ACT),maximum blood flow velocity(Vmax),left ventriculaRejection fraction(LVEF)]and immune function related indexes such as interleukin-4(IL-4),interleukin-8(IL-8),tumoRnecrosis factor-α(TNF-α)and interferon-γ(IFN-γ)were measured in the two groups before and afteRtreatment.Results:The clinical effective rate of the observation group was 90.32%,which was significantly higheRthan that of the controlled group(70.97%)(P<0.05).AfteRtreatment,the TCM symptom score and white blood cell count(WBC)in the observation group were significantly loweRthan those in the controlled group(P<0.05).Hemoglobin(Hb),platelet count(PLT)and erythrocyte count(RBC)increased significantly.AfteRtreatment,the ACT of the observation group was significantly longeRthan that of the controlled group(P<0.05).The levels of CO,Vmax and LVEF decreased significantly.AfteRtreatment,the level of IL-8 in the observation group was significantly loweRthan that in the controlled group(P<0.05).The levels of IL-4,TNF-αand IFN-γincreased significantly(P<0.05).Conclusion:Jiedu Huayu Recipe combined with Imatinib is effective to treat ALL,which can effectively relieve clinical symptoms,improve cardiovasculaRfunction and regulate immune function.
