丹参饮联合温胆汤加减治疗对稳定性心绞痛患者心肌缺血的保护机制研究

Study on the Protective Mechanism of Modified Danshen Drink Combined with Wendan Decoction Treatment on Myocardial Ischemia foRPatients with Stable Angina Pectoris

目的:探讨丹参饮联合温胆汤加减治疗对稳定性心绞痛患者心肌缺血的保护机制。方法:选取2018年7月至2019年10月于本院收治确诊的稳定型心绞痛患者142例,依据随机数表法将患者分为观察组及对照组,两组患者各71例。对照组患者行西医治疗,观察组在对照组的基础上行丹参饮联合温胆汤加减。比较两组患者治疗前及治疗8周后心绞痛发作次数评分、炎症因子及血液流变学指标等。结果:观察组患者临床总有效率显著高于对照组(χ^(2)=7.745,P=0.005);观察组患者治疗后持续时间、发作次数及疼痛程度均低于对照组(P<0.05);观察组患者治疗后全血高切粘度、血浆粘度、纤维蛋白原及全血低切粘度低于对照组(P<0.05);观察组患者治疗后细胞间粘附分子1(ICAM-1)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)及胱抑素C(CysC)水平低于对照组(P<0.05);两组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论:在西医常规治疗的基础上给予丹参饮联合温胆汤加减有利于改善患者血液流变性,抑制炎症反应,控制心绞痛发作次数,具有较好的临床疗效及安全性。

Objective:To explore the protective mechanism of modified Danshen Drink combined with Wendan Decoction treatment on myocardial ischemia foRpatients with stable angina pectoris.Methods:A total of 142 patients with stable angina pectoris admitted to our hospital from July 2018 to OctobeR2019 were selected,and the patients were divided into an observation group and a controlled group according to the random numbeRtable method,with 71 patients in each group.The controlled group received western medicine treatment,and the observation group received modified Danshen Drink combined with Wendan Decoction on the basis of the controlled group.The angina attack score,inflammatory factors and hemorheology indexes were compared between the two groups before and 8 weeks afteRtreatment.Results:The total effective rate in the observation group was significantly higheRthan that in the controlled group(χ^(2)=7.745,P=0.005).The duration,numbeRof attacks and pain degree of patients in the observation group were all loweRthan those in the controlled group(P<0.05).AfteRtreatment,the patients in the observation group had loweRwhole blood viscosity,plasma viscosity,fibrinogen and whole blood viscosity(P<0.05)than the controlled group.The levels of intercellulaRadhesion molecule 1(ICAM-1),tumoRnecrosis factoR(TNF-a),interleukin-6(IL-6)and cystatin C(CysC)in the observation group were loweRthan those in the controlled group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:On the basis of conventional western medicine treatment,the combination of Danshen Drink and Wendan Decoction is beneficial to the improvement of patients’ blood rheology,the inhibition of inflammatory response, the control of angina pectoris attacks, with good clinical efficacy and safety.
